Important issues relating to staffing levels, recruitment procedures and communications between the ward and senior levels also need highlighting. ' 'The Royal College of Nursing said: 'We will actively oppose scapegoating of any individual. We are also keen to see if all issues raised in our own evidence are fully addressed.'
