Surgical extraction of the impacted third molar is one of the commonest minor oral surgical procedures carried out in oral surgery. Problems created by the disturbances in post-extraction wound healing and physiologic sequelae of third molar surgery can significantly affect the patient's quality of life.
The study population consisted of 135 subjects that required surgical extraction of mandibular third molar under local anesthesia and met the inclusion criteria. Patients were assessed pre-operatively and post-operatively on days 1,3,5,7, and 14 using the United Kingdom Oral Health related Quality of Life questionnaire (UK-OHRQoL).
This study also showed that surgical removal of impacted teeth exerted a negative influence on patient's Quality of life (QoL) across various physical, social, psychological aspects of life. UK-OHRQoL-16 mean scores showed that severe difficulty in eating was experienced by 106 (78%) patients on postoperative day (POD) 1. The symptom however improved within the first week with only 16 (11.9%) experiencing this symptom by POD 7 and none by POD 14.
There was a deterioration in oral health related quality of life in the immediate postoperative period particularly POD 1 and 3 following third molar surgery, which slowly returned to preoperative level by 7th day. Routines such as eating, laughing and smiling, work and speech were also affected. Patients need to be informed of these symptoms after third molar removal so as to enable them prepare very well for the procedure and its sequelae.
拔除阻生第三磨牙是口腔外科最常见的小型手术之一。拔牙后伤口愈合障碍及第三磨牙手术的生理后遗症所引发的问题会显著影响患者的生活质量。
研究对象包括135名在局部麻醉下需要拔除下颌第三磨牙且符合纳入标准的受试者。术前及术后第1、3、5、7和14天使用英国口腔健康相关生活质量问卷(UK-OHRQoL)对患者进行评估。
本研究还表明,拔除阻生牙对患者生活质量(QoL)的身体、社交、心理等各个方面都产生了负面影响。UK-OHRQoL-16平均得分显示,术后第1天有106名(78%)患者进食严重困难。不过,该症状在第一周内有所改善,到术后第7天只有16名(11.9%)患者出现此症状,术后第14天则无患者出现。
第三磨牙手术后,尤其是术后第1天和第3天,口腔健康相关生活质量会下降,到第7天会慢慢恢复到术前水平。进食、大笑、微笑、工作和说话等日常活动也会受到影响。拔除第三磨牙后需要告知患者这些症状,以便他们为手术及其后遗症做好充分准备。
