Osteoarthritis is a global health problem, especially for the elderly. A good replacement for non-surgical treatments is the use of traditional medicines. We selected a revere plant (Nigella sativa L.), a widely utilized medicinal herb for the treatment of inflammatory conditions, from the Iranian traditional medicine (ITM) pharmacopoeia with proven anti-inflammatory and analgesic actions.
We performed a prospective, randomized, double-blind, and placebo-controlled clinical trial, in order to investigate whether the herb is useful in alleviating the symptoms of knee osteoarthritis. American College of Rheumatology clinical criteria were the basis of diagnosis, while the Knee injury and Osteoarthritis Outcome Score (KOOS) questionnaire was considered as the main outcome measure. One hundred and ten eligible patients were assigned to receive a placebo or an active intervention (2 g/day of processed N. sativa seed powder in divided doses). Acetaminophen tablets were the rescue medicine. Finally, 40 patients in the placebo group and 37 patients in the active group completed the trial and were included in the statistical analysis.
Both cohorts demonstrated statistically significant within-group differences (P < 0.05) in some subscales that were more prominent in the active group without any considerable adverse effects. Nevertheless, KOOS score results and the mean number of acetaminophen tablets used by patients showed no statistically significant between-group differences.
It can be concluded that future programmed studies with larger sample sizes, longer follow-up periods, and other forms of N. sativa seeds as an active intervention is necessary to evaluate its efficacy in relieving the symptoms of knee osteoarthritis.
骨关节炎是一个全球性的健康问题,对老年人尤为如此。传统药物是一种很好的非手术治疗替代方法。我们从伊朗传统医学药典中选取了一种备受推崇的植物(黑种草),它是一种广泛用于治疗炎症性疾病的药用草本植物,具有已证实的抗炎和止痛作用。
我们进行了一项前瞻性、随机、双盲和安慰剂对照的临床试验,以研究这种草本植物是否有助于缓解膝关节骨关节炎的症状。美国风湿病学会临床标准是诊断的依据,而膝关节损伤和骨关节炎疗效评分(KOOS)问卷被视为主要结局指标。110名符合条件的患者被分配接受安慰剂或积极干预(每天2克经加工的黑种草籽粉,分剂量服用)。对乙酰氨基酚片为急救药物。最后,安慰剂组的40名患者和积极治疗组的37名患者完成了试验并纳入统计分析。
两个队列在一些分量表上均显示出具有统计学意义的组内差异(P<0.05),在积极治疗组中更为显著,且没有任何明显的不良反应。然而,KOOS评分结果以及患者使用对乙酰氨基酚片的平均数量在组间差异无统计学意义。
可以得出结论,未来有必要进行样本量更大、随访期更长以及采用其他形式的黑种草籽作为积极干预措施的规划性研究,以评估其缓解膝关节骨关节炎症状的疗效。
