Lymphomas arising in the central nervous system (CNS) of immunocompromised hosts are most commonly non-Hodgkin's lymphomas and are highly associated with Epstein-Barr virus (EBV). Here we report an autopsy case of EBV-associated CNS diffuse large B-cell lymphoma (DLBCL) in a host suffering from systemic lupus erythematosus who underwent immunosuppressive therapy. After autopsy, EBV-associated CNS DLBCL as well as pulmonary mixed aspergillosis and pneumonia were added to the cause of clinical manifestations of complicated pneumonia and cerebral hemorrhage in this immunocompromised patient. In conclusion, complex disease processes were revealed by autopsy in this case, indicating that the clinicopathological correlations observed through autopsy can improve our understanding of disease progression and contribute to the management of similar patients in the future.
免疫功能低下宿主中枢神经系统(CNS)发生的淋巴瘤最常见的是非霍奇金淋巴瘤,且与爱泼斯坦-巴尔病毒(EBV)高度相关。在此,我们报告一例接受免疫抑制治疗的系统性红斑狼疮患者发生EBV相关的中枢神经系统弥漫性大B细胞淋巴瘤(DLBCL)的尸检病例。尸检后发现,该免疫功能低下患者复杂肺炎和脑出血的临床表现病因中增加了EBV相关的中枢神经系统DLBCL以及肺部混合曲霉菌病和肺炎。总之,该病例尸检揭示了复杂的疾病过程,表明通过尸检观察到的临床病理相关性可增进我们对疾病进展的理解,并有助于未来对类似患者的管理。
