Abstract

BACKGROUND

Weight gain is a common adverse effect of sodium valproic acid (VPA) in children with epilepsy. Several mechanisms of VPA-induced obesity have been suggested such as increased appetite, facultative thermogenesis, and elevated insulin and leptin levels. In this study, we aimed to investigate the role of Insulin and Leptin in the pathogenesis of weight gain caused by VPA.

MATERIALS AND METHODS

Body mass index (BMI) was calculated, and serum insulin and leptin levels were measured in 45 consecutive patients and 45 controls.

RESULTS

The mean BMI of the cases and control group was 22.97 kg/m and 19.4 kg/m, respectively, and it was significantly higher in cases ( < 0.001). Fasting serum insulin levels were higher in VPA group (26.3 μU/ml) than in controls (15.83 μU/ml), which was statistically significant ( < 0.001). Serum leptin levels were also found to be elevated significantly in VPA group (7.9 ng/ml) than in controls (1.6 ng/ml).

CONCLUSION

Sodium VPA is associated with significant rise of BMI, hyperinsulinemia, raised insulin resistance, and increased leptin levels in children with epilepsy.

材料与方法

计算45例连续患者和45例对照的体重指数(BMI),并测量血清胰岛素和瘦素水平。

结果

病例组和对照组的平均BMI分别为22.97kg/m²和19.4kg/m²,病例组显著更高(P<0.001)。VPA组空腹血清胰岛素水平(26.3μU/ml)高于对照组(15.83μU/ml),差异有统计学意义(P<0.001)。VPA组血清瘦素水平(7.9ng/ml)也显著高于对照组(1.6ng/ml)。

结论

丙戊酸钠与癫痫患儿BMI显著升高、高胰岛素血症、胰岛素抵抗增加及瘦素水平升高有关。
