We successfully treated a patient who was diagnosed of having hypertrophic obstructive cardiomyopathy after the aortic valve replacement surgery for the concomitant aortic stenosis. We report this first in kind new procedure exclusively developed in our center, Liwen procedure (percutaneous intramyocardial septal radiofrequency ablation), for patients with left ventricular outflow tract obstruction in spite of maximal medical therapy. The procedure was performed under transthoracic echo guidance. We discussed the technical details, safety, and effectiveness with corresponding images. The patient did well one year after the procedure without LVOT obstruction or arrhythmia.
我们成功治疗了一名在因合并主动脉瓣狭窄行主动脉瓣置换术后被诊断为肥厚性梗阻性心肌病的患者。我们首次报告了在我们中心专门研发的一种全新手术——立文手术（经皮心肌间隔射频消融术），用于尽管接受了最大程度药物治疗但仍存在左心室流出道梗阻的患者。该手术在经胸超声引导下进行。我们结合相应图像讨论了技术细节、安全性和有效性。该患者术后一年情况良好，未出现左心室流出道梗阻或心律失常。
