• Mediastinal hematoma is a rare but life-threatening complication after thrombolysis. • It can be rapidly identified on transthoracic echocardiography. • Management of anticoagulation is difficult if hemorrhagic complications occur. • Ultrasound is a useful tool for detection and management of complications.
