We present a case of disseminated Kaposi's sarcoma with both cutaneous and extracutaneous involvement in an HIV-infected patient with a relatively high CD4 count of 369 cell/mm. He developed chronic diarrhea, constitutional symptoms, worsening bilateral pleural effusion with respiratory distress, and progression of skin lesions distributed over his chest and extremities. The temporal relationship between rapid clinical progression and initiation of HAART suggested the possibility of Kaposi's sarcoma-associated immune reconstitution inflammatory syndrome, which eventually resulted in the death of this patient.
我们报告一例艾滋病病毒感染患者,其CD4细胞计数相对较高,为369个细胞/mm³,患有播散性卡波西肉瘤,累及皮肤和皮肤外部位。他出现了慢性腹泻、全身症状、双侧胸腔积液加重伴呼吸窘迫,以及胸部和四肢皮肤病变进展。快速临床进展与高效抗逆转录病毒治疗(HAART)开始之间的时间关系提示存在卡波西肉瘤相关免疫重建炎症综合征的可能性,该患者最终死亡。
