Abstract

OBJECTIVES

To evaluate and compare the diagnostic potential of F-fluorodeoxyglucose positron emission tomography/magnetic resonance imaging ( FDG-PET/MRI) and MRI for recurrence diagnostics after primary therapy in patients with adenoid cystic carcinoma (ACC).

METHODS

A total of 32 dedicated head and neck F-FDG PET/MRI datasets were included in this analysis. MRI and F-FDG PET/MRI datasets were analyzed in separate sessions by two readers for tumor recurrence or metastases.

RESULTS

Lesion-based sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy were 96%, 84%, 90%, 93%, and 91% for F-FDG PET/MRI and 77%, 94%, 95%, 73%, and 84% for MRI, resulting in a significantly higher diagnostic accuracy of F-FDG PET/MRI compared to MRI (P < .005).

CONCLUSION

F-FDG PET/MRI is superior to MRI in detecting local recurrence and metastases in patients with ACC of the head and neck. Especially concerning its negative predictive value, F-FDG PET/MRI outperforms MRI.

目的

评估并比较氟代脱氧葡萄糖正电子发射断层扫描/磁共振成像(FDG-PET/MRI)与 MRI 在前瞻性治疗后对头颈腺样囊性癌(ACC)患者复发诊断中的应用价值。

方法

本研究共纳入 32 例头颈部专用 F-FDG PET/MRI 数据集。由两位阅片者分别在单独的阅片时段内对 MRI 和 FDG-PET/MRI 数据集进行分析,以评估肿瘤复发或转移情况。

结果

基于病灶的灵敏度、特异性、阳性预测值、阴性预测值和诊断准确率分别为 FDG-PET/MRI 组 96%、84%、90%、93%和 91%,MRI 组 77%、94%、95%、73%和 84%,提示 FDG-PET/MRI 的诊断准确率显著高于 MRI(P<0.005)。

结论

FDG-PET/MRI 较 MRI 更有助于检测头颈部 ACC 患者的局部复发和远处转移。特别是在阴性预测值方面,FDG-PET/MRI 优于 MRI。
