Successful treatment of gonorrhea has always been jeopardized by the emergence of resistance to antibiotics recommended as first-line therapies. The present investigation was carried out to demonstrate the current status of antimicrobial susceptibility of with a special reference to azithromycin and ceftriaxone.
Microscopical detection in Gram-stained smear and isolation by culture in Thayer-Martin medium were done for 60 clinically suspected gonorrhea patients using urethral discharge or prostatic secretion for male and endocervical secretion for female. Isolates of were subjected to antimicrobial susceptibility testing by modified Kirby Bauer disk diffusion method against eight antimicrobial drugs including azithromycin and ceftriaxone.
Culture yielded a total of 25(42%) isolates of from 60 clinically suspected patients of both sexes; 21 from male (17 from urethral discharge and 04 from prostatic secretion) and 04 from female (endocervical secretion). Isolates of showed moderate to high resistance (60 to 88%) to penicillin, tetracycline, cotrimoxazole, erythromycin, ciprofloxacin and cefixime. While resistance to azithromycin and ceftriaxone was 60% and 48% respectively, which was also moderate.
Our findings indicate moderate to the high resistance of to conventional antibiotics. It also showed moderate resistance to azithromycin and ceftriaxone, current dual therapy recommended by the WHO for the treatment of genital gonorrhea, which is alarming.
淋病的成功治疗一直受到对推荐作为一线治疗药物的抗生素产生耐药性的威胁。本研究旨在阐明淋病奈瑟菌抗菌药物敏感性的现状,特别提及阿奇霉素和头孢曲松。
对60例临床疑似淋病患者进行革兰氏染色涂片显微镜检测,并使用男性尿道分泌物或前列腺分泌物以及女性宫颈分泌物在Thayer-Martin培养基中进行培养分离。采用改良的Kirby Bauer纸片扩散法对淋病奈瑟菌分离株进行包括阿奇霉素和头孢曲松在内的8种抗菌药物的药敏试验。
从60例临床疑似淋病的男女患者中共培养出25株(42%)淋病奈瑟菌分离株;男性21株(尿道分泌物17株,前列腺分泌物4株),女性4株(宫颈分泌物)。淋病奈瑟菌分离株对青霉素、四环素、复方新诺明、红霉素、环丙沙星和头孢克肟表现出中度至高耐药性(60%至88%)。而对阿奇霉素和头孢曲松的耐药率分别为60%和48%,也为中度耐药。
我们的研究结果表明淋病奈瑟菌对传统抗生素具有中度至高耐药性。它对世界卫生组织目前推荐用于治疗生殖器淋病的联合疗法阿奇霉素和头孢曲松也表现出中度耐药性,这令人担忧。
