Abstract

OBJECTIVE

To observe the therapeutic effect of electroacupuncture (EA) and acupoint catgut embedding in the treatment of simple obesity.

METHODS

Simple obesity patients were randomized into EA group (7 men and 36 women, 21-42 years in age) and catgut embedding group (4 men and 37 women, 22-41 years in age). EA (4 Hz/20 Hz, a tolerable strength) was applied to main acupoints Zhongwan (CV12), bilateral Tianshu (ST25), Daheng (SP15), Daimai (GB26), Shuidao (ST28), Zhigou (TE6), Yinlingquan (SP9), Zusanli (ST36), Fenglong (ST40), and Sanyinjiao (SP6), and some auxiliary acupoints for 30 min, once every other day for 30 times. Subcutaneous catgut-embedment was performed in the same acupoints. Nine to 11 acupoints were used every time, once every 10 days for 6 times. Before and after the treatment, fasting serum leptin and insulin (INS) contents were detected by radioimmunoassay, and the correlation between the leptin, INS and the body mass index (BMI) was analyzed, respectively.

RESULTS

Following the treatment, the serum leptin and INS concentrations and BMI in both groups were significantly decreased in comparison with those of their own pre-treatment (<0.01). No significant differences were found between the two groups in the levels of serum leptin and INS after the treatment (>0.05). There were positive correlations between the decreased BMI and serum leptin/INS contents in both EA and catgut embedding groups (<0.01).

CONCLUSION

Both catgut embedding and EA interventions have a positive effect in reducing body weight of simple obesity patients, which may be related to its effects in down-regulating serum leptin and INS levels and in correcting leptin resistance and insulin resistance.

目的

观察电针(EA)与穴位埋线治疗单纯性肥胖症的疗效。

方法

将单纯性肥胖患者随机分为电针组(男7例,女36例,年龄21 - 42岁)和埋线组(男4例,女37例,年龄22 - 41岁)。电针(频率4Hz/20Hz,强度以患者耐受为度)取中脘(CV12)、双侧天枢(ST25)、大横(SP15)、带脉(GB26)、水道(ST28)、支沟(TE6)、阴陵泉(SP9)、足三里(ST36)、丰隆(ST40)、三阴交(SP6)为主穴,酌配一些辅助穴位,留针30分钟,隔日1次,共30次。穴位埋线取相同穴位,每次选9 - 11个穴位,每10天1次,共6次。治疗前后采用放射免疫法检测空腹血清瘦素和胰岛素(INS)含量,并分别分析瘦素、INS与体重指数(BMI)之间的相关性。

结果

治疗后,两组患者血清瘦素、INS浓度及BMI均较治疗前显著降低(<0.01)。治疗后两组血清瘦素和INS水平比较,差异无统计学意义(>0.05)。电针组和埋线组BMI降低值与血清瘦素/INS含量均呈正相关(<0.01)。

结论

穴位埋线和电针干预对单纯性肥胖患者体重减轻均有积极作用,其机制可能与下调血清瘦素和INS水平、纠正瘦素抵抗和胰岛素抵抗有关。
