Cauda equina paragangliomas are rare neuroendocrine benign and slow-growing tumors. Acute paraplegia, occurring because of sudden intratumoral hemorrhage, represents an extremely rare clinical picture of this disease. We report the case of a 64-year-old male presenting with a 5-day acute lower back pain, sciatica, and leg weakness. Spinal imaging showed an intradural mass of the cauda equina region and an emergent surgical treatment was achieved. The lesion was removed "," and subarachnoid blood was noticed during surgery. The postoperative course was uneventful, with complete regression of pain and progressive motor recovery. The histological study revealed typical microscopic and immunohistochemical features of paragangliomas.
马尾神经节细胞瘤是罕见的神经内分泌良性且生长缓慢的肿瘤。因肿瘤内突然出血而发生的急性截瘫是这种疾病极其罕见的临床表现。我们报告一例64岁男性病例,其出现5天的急性下背部疼痛、坐骨神经痛和腿部无力。脊柱影像学检查显示马尾神经区域硬膜内有肿块,并进行了紧急手术治疗。病变被切除,手术过程中发现蛛网膜下腔出血。术后过程顺利,疼痛完全消退,运动功能逐渐恢复。组织学研究显示神经节细胞瘤具有典型的微观和免疫组化特征。
