This study aims to investigate the effects of a home-based exercise program in combination with paraffin therapy on pain, functional status, grip strength, and quality of life (QoL) and to compare the outcomes of these two treatment modalities.
This prospective, single-blind, randomized-controlled study included a total of 61 patients (8 males, 53 females, mean age 59.22 years; range, 35 to 78 years) who were diagnosed with hand osteoarthritis (HOA) according to the American College of Rheumatology criteria between November 2016 and February 2017. The first group (group 1) (n=31) received paraffin therapy + home-based exercise program and the second group (group 2) (n=30) received home-based exercise program alone. All patients were assessed using the Visual Analog Scale (VAS), Australian/Canadian (AUSCAN) Osteoarthritis Hand Index, Health Assessment Questionnaire (HAQ), Hand Grip Strength (HGS), and Finger Pinch Strength (FPS) at baseline and at two and six weeks after the intervention.
In group 1, there were statistically significant improvements in all parameters at two and six weeks (p<0.05). Statistically significant differences were observed in the HGS and AUSCAN Osteoarthritis Hand Index scores in group 2 at two and six weeks (p<0.05). Inter-group analysis showed statistically significant differences in favor of group 1 in the VAS, HAQ, AUSCAN Osteoarthritis Hand Index, HGS, and FPS scores at two and six weeks compared to baseline scores (p<0.05).
Paraffin therapy combined with home-based exercise program in patients with symptomatic HOA provides effective pain control and is effective in improving functional status, HGS, FPS, and QoL in short term.
本研究旨在探讨居家锻炼计划联合石蜡疗法对疼痛、功能状态、握力和生活质量(QoL)的影响,并比较这两种治疗方式的效果。
本前瞻性、单盲、随机对照研究共纳入61例患者(8例男性,53例女性,平均年龄59.22岁;范围35至78岁),这些患者在2016年11月至2017年2月期间根据美国风湿病学会标准被诊断为手部骨关节炎(HOA)。第一组(第1组)(n = 31)接受石蜡疗法+居家锻炼计划,第二组(第2组)(n = 30)仅接受居家锻炼计划。所有患者在基线以及干预后两周和六周时使用视觉模拟量表(VAS)、澳大利亚/加拿大(AUSCAN)骨关节炎手部指数、健康评估问卷(HAQ)、握力(HGS)和捏力(FPS)进行评估。
在第1组中,两周和六周时所有参数均有统计学显著改善(p < 0.05)。在第2组中,两周和六周时HGS和AUSCAN骨关节炎手部指数评分有统计学显著差异(p < 0.05)。组间分析显示,与基线评分相比,在两周和六周时,第1组在VAS、HAQ、AUSCAN骨关节炎手部指数、HGS和FPS评分方面有统计学显著优势(p < 0.05)。
对于有症状的HOA患者,石蜡疗法联合居家锻炼计划可有效控制疼痛,并在短期内有效改善功能状态、HGS、FPS和QoL。
