A case of haemorrhage due to oesophageal varices in a cirrhotic-portal hypertense patient successfully treated with side-to-side portocaval anastomosis is reported. Long-term follow-up showed that the surgical result was satisfactory. On the basis of this case, the problems and current tendencies towards derivative operations in the prophylaxis and treatment of portal hypertension complications are reassessed.
