Closed rupture of the anterior tibial tendon is uncommon, with only a few cases being documented in literature. It usually happens in the forceful plantar flexion of the foot while in eversion and the simultaneous contraction of the anterior tibialis muscle.
We present the case of a 65-year-old man with closed rupture of the tibialis anterior tendon after injury who was treated by primary surgical repair. The surgical technique as well as the rehabilitation protocol are described in detail.
Primary surgical reconstruction repair is indicated in younger, active individuals. It appears that early diagnosis and treatment provides the best clinical results, while delayed treatment usually requires the use of a tendon graft in combination with tendon transfers, giving inferior results compared to primary tendon repair.
胫前肌腱闭合性断裂并不常见，文献中仅有少数病例记载。它通常发生在足部强力跖屈伴外翻且胫前肌同时收缩时。
我们报告一例65岁男性，受伤后发生胫前肌腱闭合性断裂，接受了一期手术修复治疗。详细描述了手术技术及康复方案。
对于年轻、活动量较大的个体，建议进行一期手术重建修复。早期诊断和治疗似乎能带来最佳临床效果，而延迟治疗通常需要使用肌腱移植并结合肌腱转移，与一期肌腱修复相比效果较差。
