BACKGROUND/AIMS: To investigate whether the association of long-term intraocular pressure (IOP) fluctuation with the rate of progression of normal-tension glaucoma (NTG) differs between myopia and non-myopia.
The medical records of 65 myopic NTG (axial length (AL) > 24.0 mm) and 64 non-myopic NTG eyes (AL < 24.0 mm), who had been treated with topical medications for more than 5 years, were reviewed. Multiple linear regression models were fitted to analyse the relationships of the slope of mean deviation (MD) or visual field index (VFI) with the clinical factors, including the interactions with myopia.
The average follow-up period was 8.3 years. Twenty-two (22) non-myopic eyes (34.4%) and 27 myopic eyes (41.5%) showed NTG progression (p=0.511). The interaction of myopia with IOP fluctuation was a significant factor regarding both MD and VFI slope (p=0.002, 0.024, respectively); stratified analyses suggested that the risk effect of IOP fluctuation was significant only in myopic NTG in terms of both MD (β= -1.27, p=0.003) and VFI slope (β=-2.32, p=0.011).
Long-term IOP fluctuation was significantly related to faster visual field progression in myopic NTG eyes, compared with non-myopic NTG eyes.
背景/目的:研究近视与非近视患者中,长期眼压(IOP)波动与正常眼压性青光眼(NTG)进展速率之间的关联是否存在差异。
回顾了65例近视性NTG(眼轴长度(AL)> 24.0 mm)和64例非近视性NTG患眼(AL < 24.0 mm)的病历,这些患者接受局部药物治疗超过5年。采用多元线性回归模型分析平均偏差(MD)斜率或视野指数(VFI)与临床因素之间的关系,包括与近视的相互作用。
平均随访期为8.3年。22例(34.4%)非近视患眼和27例(41.5%)近视患眼出现NTG进展(p = 0.511)。近视与IOP波动的相互作用是影响MD和VFI斜率的重要因素(分别为p = 0.002,0.024);分层分析表明,就MD(β = -1.27,p = 0.003)和VFI斜率(β = -2.32,p = 0.011)而言,IOP波动的风险效应仅在近视性NTG中显著。
与非近视性NTG患眼相比,长期IOP波动与近视性NTG患眼的视野进展更快显著相关。
