Abstract

This study aims to investigate the diagnostic and prognostic values of EpCAM, TGM2, and HE4 in endometrial cancer (EC). In this study, 42 patients diagnosed with EC (EC group), 41 patients diagnosed with myoma (benign group), and 43 healthy women (healthy group), who applied to Affiliated Hospital of Xuzhou Medical University between March 2018 - September 2019 were recruited. Serum EpCAM, TGM2, and IL-33 levels were measured by ELISA, while serum HE4 and CA-125 levels were measured by ECLIA. The serum markers listed above were also measured in 12 paired pre- and post-operative EC patients. The diagnostic and prognostic values of serum markers were analyzed. The serum EpCAM, TGM2, HE4, CA-125, and IL-33 levels were significantly higher in the EC group. The sensitivity and specificity of combined detection of EpCAM and HE4 was 92.86 and 69.05%, which were significantly higher than using a single marker or other combinations. Among these markers, serum HE4 levels were significantly higher in patients with myometrial invasion, metastasis, and lymphovascular invasion ( = 0.006, = 0.0004, = 0.0004, respectively). And serum TGM2 levels were significantly decreased in post-operative than that of pre-operative EC patients ( < 0.001). The combination of EpCAM and HE4 showed the highest specificity and sensitivity in the diagnosis of EC. HE4 was successful in the detection of high-risk individuals preoperatively. Additionally, TGM2 might be a prognostic factor for EC.

本研究旨在探讨上皮细胞粘附分子(EpCAM)、转谷氨酰胺酶2(TGM2)和人附睾蛋白4(HE4)在子宫内膜癌(EC)中的诊断和预后价值。本研究招募了2018年3月至2019年9月期间就诊于徐州医科大学附属医院的42例确诊为EC的患者(EC组)、41例确诊为肌瘤的患者(良性组)和43例健康女性(健康组)。采用酶联免疫吸附测定法(ELISA)检测血清EpCAM、TGM2和白细胞介素-33(IL-33)水平,采用电化学发光免疫分析法(ECLIA)检测血清HE4和糖类抗原125(CA-125)水平。还对12例EC患者术前和术后配对样本进行了上述血清标志物检测。分析血清标志物的诊断和预后价值。EC组血清EpCAM、TGM2、HE4、CA-125和IL-33水平显著升高。EpCAM和HE4联合检测的敏感性和特异性分别为92.86%和69.05%,显著高于单一标志物或其他组合。在这些标志物中,肌层浸润、转移和淋巴血管浸润患者的血清HE4水平显著升高(分别为P = 0.006、P = 0.0004、P = 0.0004)。与术前相比,EC患者术后血清TGM2水平显著降低(P < 0.001)。EpCAM和HE4联合检测在EC诊断中显示出最高的特异性和敏感性。HE4在术前成功检测出高危个体。此外,TGM2可能是EC的一个预后因素。
