Tuboovarian sigmoid fistulae are rare complications of pelvic inflammatory disease. A 31-year-old woman had a severe post-cesarean-section infection that formed a tuboovarian abscess and ruptured into the sigmoid colon. The abscess and subsequent fistula led to persistent pain and purulent diarrhea. A salpingo-oophorectomy with closure of the colon was performed. The patient was still asymptomatic 20 months after the surgery.
