To evaluate the effect of transcutaneous electrical nerve stimulation (C-TENS) in the rehabilitation of rotator cuff injury.
A total of 60 patients diagnosed with mild to moderate rotator cuff injury were randomly divided into the control group and test group. Both groups received conventional rehabilitation treatment including physical therapy, joint mobilization, interventional therapy, and family recovery training. The test group additionally received TENS treatment on the upper area 4 or 5 of the wrist, based on the wrist-ankle acupuncture (WAA) theory. The results of the visual analogue scale (VAS), shoulder range of motion (ROM), and Constant-Murley score (CMs) were collected before and after the 1 treatment and after the 5 treatment. The differences of those indicators between the two groups were analyzed statistically.
The VAS scores of measurement points after treatment were all improved compared with those at baseline. There was a significant difference between the two groups after the 1 and the 5 treatment ( < 0.05), and the improvements in the test group were better than those in the control group. The ROM of flexion, extension, abduction, adduction, internal rotation, and external rotation after the 1 treatment and 5 treatment in both groups were all improved compared with those at baseline. There was no significant difference between two groups. The CMs of the two groups after the 5 treatment were all improved compared with those at baseline. There was no significant difference between two groups. No adverse events occurred during the treatment.
Electrical stimulation on the wrist combined with conventional rehabilitation is more effective in relieving pain than the conventional rehabilitation alone. Electrical stimulation on the wrist combined with conventional rehabilitation has no obvious effect in improving shoulder joint mobility and shoulder function.
评估经皮神经电刺激(C-TENS)在肩袖损伤康复中的作用。
将 60 例轻中度肩袖损伤患者随机分为对照组和试验组。两组均接受常规康复治疗,包括物理治疗、关节松动术、介入治疗和家庭康复训练。试验组在此基础上,根据腕踝针(WAA)理论,在上腕 4 或 5 区进行 TENS 治疗。分别于治疗前、治疗 1 次后、治疗 5 次后,采用视觉模拟评分(VAS)、肩关节活动度(ROM)和 Constant-Murley 评分(CMs)评估两组患者的疗效。对两组间各指标的差异进行统计学分析。
治疗后各时点的 VAS 评分均较治疗前改善,两组间差异有统计学意义( < 0.05),且试验组改善优于对照组。两组治疗后各关节ROM(前屈、后伸、外展、内收、内旋、外旋)较治疗前均有改善,两组间差异无统计学意义。两组治疗后 CMs 均较治疗前改善,两组间差异无统计学意义。治疗过程中均未发生不良反应。
腕部电刺激联合常规康复治疗比单纯常规康复治疗更能有效缓解疼痛。腕部电刺激联合常规康复治疗对改善肩关节活动度和肩关节功能无明显作用。
