Abstract

OBJECTIVE

The purpose of this study was to compare the efficacy of focused ultrasound (FU) and interferon drug therapy for cervical intraepithelial neoplasia 1 (CIN1) and chronic cervicitis associated with high risk human papillomavirus (HR-HPV) infection, as well as analyze the influencing factors.

METHODS

A retrospective cohort study was performed from January 2017 to December 2019. A total of 592 patients were enrolled, of which 300 patients were treated with FU and 292 patients were treated with interferon drugs. Kaplan-Meier curves and a COX regression model were used to compare the curative effects of the two therapeutic methods using HR-HPV clearance as the main outcome. The relationship between age, HR-HPV infection type, pathological type, preoperative HR-HPV status and HR-HPV clearance were also analyzed.

RESULTS

The median time for HR-HPV clearance was 6.00 months (95% CI: 5.24-6.76) in the FU group and 26.00 months (95% CI: 22.32-29.68) in the medication group. A significant difference was observed between the two groups (χ =198.902,  = 0.000). The HR-HPV clearance rate was 4.927 (95% CI 3.840-6.321;  = 0.000) times higher in the patients treated with FU than those treated with interferon drugs. In the FU group, no significant difference was observed in HR-HPV clearance rate between CIN1 and chronic cervicitis (χ=0.660,  = 0.416), which was also insignificant between HR-HPV persistent and non-persistent infections (χ=0.751,  = 0.386).

CONCLUSION

FU therapy can eliminate HR-HPV infections in a short period of time. Moreover, the treatment efficacy of FU was significantly superior to that of interferon drugs.

本研究旨在比较聚焦超声(FU)与干扰素药物治疗宫颈上皮内瘤变 1 级(CIN1)和慢性宫颈炎合并高危型人乳头瘤病毒(HR-HPV)感染的疗效,并分析影响因素。

方法

回顾性队列研究,于 2017 年 1 月至 2019 年 12 月纳入 592 例患者,FU 组 300 例,干扰素组 292 例。以 HR-HPV 清除为主要结局,采用 Kaplan-Meier 曲线和 COX 回归模型比较两种治疗方法的疗效。分析年龄、HR-HPV 感染类型、病理类型、术前 HR-HPV 状态与 HR-HPV 清除的关系。

结果

FU 组 HR-HPV 清除中位时间为 6.00 个月(95%CI:5.24-6.76),药物组为 26.00 个月(95%CI:22.32-29.68),两组比较差异有统计学意义(χ=198.902, = 0.000)。FU 组 HR-HPV 清除率是干扰素组的 4.927 倍(95%CI 3.840-6.321; = 0.000)。FU 组中,CIN1 与慢性宫颈炎患者 HR-HPV 清除率差异无统计学意义(χ=0.660,  = 0.416),HR-HPV 持续感染与非持续感染患者 HR-HPV 清除率差异也无统计学意义(χ=0.751,  = 0.386)。

结论

FU 治疗可在短时间内清除 HR-HPV 感染,且疗效明显优于干扰素药物。
