COVID-19 Knowledge and Pandemic-Associated Distress Among the Hospital Pharmacist Workforce in China.

Abstract

BACKGROUND

The unprecedented disruption brought about by the global coronavirus disease 2019 (COVID-19) pandemic had produced tremendous influence on the practice of pharmacy. Sufficient knowledge of pharmacists was needed to deal with the epidemic situation; however, outbreak also aggravated psychological distress among health-care professionals. Therefore, this study aimed to determine knowledge about the pandemic and related factors, prevalence and factors associated with psychological distress among hospital pharmacists of Xinjiang Province, China.

METHODS

An anonymous online questionnaire-based cross-sectional study was conducted by means of WeChat, a popular social media platform in China, February 23-27, 2020, during the COVID-19 outbreak. The survey questionnaire consisted of 4 parts, including informed consent section, demographic section, knowledge about COVID-19, and assessment of overall mental health through World Health Organization's Self-Reporting Questionnaire (SRQ-20). A score of 8 or above on SRQ-20 was used as cutoff to classify the participant as in psychological distress. SRQ-20 score and related knowledge score were used as dependent variables, demographic characteristics (such as gender, age, monthly income, etc.) were used as independent variables, and univariate binary logistic regression was used to screen out the variables with < 0.05. Then, the filtered variables were used as independent variables, and multivariate logistic regression models were used to analyze associations with sufficient knowledge of COVID-19 and psychological distress.

RESULTS

A total of 365 pharmacists participated in the survey, fewer than half (35.1%; = 128) of pharmacists attained a score of 6 or greater (out of 10) in overall disease knowledge, and most were able to select effective disinfectants and isolation or discharge criteria. In the multivariable model, age ages 31-40 (odds ratio [OR] = 3.25; < 0.05), ages 41-50 (OR = 2.96; < 0.05) versus >50 (referent); primary place of practice in hospitals: drug supply (OR = 4.00; < 0.01), inpatient pharmacy (OR = 2.06, < 0.01), clinical pharmacy (OR = 2.17, < 0.05) versus outpatient pharmacy (referent); monthly income Renminbi (RMB, China's legal currency) 5000-10,000 (OR = 1.77; < 0.05) versus < 5000 (referent); contact with COVID-19 patients or suspected cases (OR = 2.27; < 0.01); access to COVID-19 knowledge remote work+ on-site work (OR = 6.07; < 0.05), single on-site work (OR = 6.90; < 0.01) versus remote work (referent) were related to better knowledge of COVID-19. Research found that 18.4% of pharmacists surveyed met the SRQ-20 threshold for distress. Self-reported history of mental illness (OR = 3.56; < 0.05) and working and living in hospital versus delay in work resumption (OR = 2.87; < 0.01) were found to be risk factors of psychological distress.

CONCLUSIONS

Further training of COVID-19 knowledge was required for pharmacists. As specific pharmacist groups were prone to psychological distress, it was important for individual hospitals and government to consider and identify pharmacists' needs and take steps to meet their needs with regard to pandemic and other work-related distress.

方法

于2020年2月23日至27日COVID-19疫情期间,通过中国流行的社交媒体平台微信进行了一项基于匿名在线问卷的横断面研究。调查问卷包括4个部分,即知情同意部分、人口统计学部分、关于COVID-19的知识,以及通过世界卫生组织自填问卷(SRQ-20)评估总体心理健康状况。SRQ-20得分8分及以上作为将参与者分类为有心理困扰的临界值。将SRQ-20得分和相关知识得分作为因变量,人口统计学特征(如性别、年龄、月收入等)作为自变量,采用单因素二元逻辑回归筛选出P<0.05的变量。然后,将筛选出的变量作为自变量,采用多因素逻辑回归模型分析与对COVID-19的充分了解和心理困扰的关联。

结果

共有365名药剂师参与了调查,不到一半(35.1%;n = 128)的药剂师在总体疾病知识方面(满分10分)得分为6分或更高,且大多数人能够选择有效的消毒剂以及隔离或出院标准。在多变量模型中,年龄31 - 40岁(比值比[OR]=3.25;P<0.05)、41 - 50岁(OR = 2.96;P<0.05)与>50岁(参照组)相比;主要执业地点在医院:药品供应部门(OR = 4.00;P<0.01)、住院药房(OR = 2.06,P<0.01)、临床药房(OR = 2.17,P<0.05)与门诊药房(参照组)相比;月收入人民币(中国法定货币)5000 - 10,000元(OR = 1.77;P<0.05)与<5000元(参照组)相比;接触COVID-19患者或疑似病例(OR = 2.27;P<0.01);获取COVID-19知识远程工作+现场工作(OR = 6.07;P<0.05)、单纯现场工作(OR = 6.90;P<0.01)与远程工作(参照组)与对COVID-19有更好的了解有关。研究发现,18.4%的接受调查药剂师达到了SRQ-20困扰临界值。自我报告的精神疾病史(OR = 3.56;P<0.05)以及在医院工作和生活与复工延迟(OR = 2.87;P<0.01)被发现是心理困扰的危险因素。

结论

药剂师需要进一步接受COVID-19知识培训。由于特定药剂师群体容易出现心理困扰,各医院和政府考虑并识别药剂师的需求,并采取措施满足他们在疫情及其他与工作相关困扰方面的需求非常重要。
