This study aimed to evaluate whether the posterior cruciate ligament sacrifice during total knee arthroplasty (TKA) has any effect on postoperative standing balance or not.
The patients who underwent bilateral TKA with either CR or PS design were analyzed. 30 patients (10 PS, 20 CR) were included for the final analysis. TKA designs were compared in terms of Lysholm score, range of motion, and balance characteristics including somatosensorial, vestibular, and visual balance scales, adaptation, limits of stability, and weight-bearing/squat tests by computerized dynamic posturography.
The mean follow-up time was 59 months for CR, 49 months for PS group. The average Lysholm score values were 94 for CR and 95 for PS group, indicating functionally similar patient groups. The average knee flexion was found significantly higher in PS group (114°) compared to CR group (102°) ( = 0.009). In the CR group, motor adaptation tests (toes up/toes down) were found to be better ( = 0.034). In the on-axis velocity parameter (linear goal orientation) of limits of stability test, PS group patients were found to be more successful ( = 0.035).
The use of CR implants can be recommended in patients with a high risk of falling since they provide better motor adaptation providing rapid reactions to rapid surface changes. Better linear goal orientation in PS group, providing a faster movement in an intended direction, should be considered when planning the ideal implant for the patients with relevant activities.
本研究旨在评估全膝关节置换术(TKA)中后交叉韧带切除对术后站立平衡的影响。
分析接受 CR 或 PS 设计双侧 TKA 的患者。最终分析纳入 30 例患者(10 例 PS,20 例 CR)。从 Lysholm 评分、运动范围以及平衡特征(包括本体感觉、前庭和视觉平衡量表、适应能力、稳定性极限和计算机动态姿势图的负重/深蹲测试)方面比较 TKA 设计。
CR 组的平均随访时间为 59 个月,PS 组为 49 个月。CR 组和 PS 组的平均 Lysholm 评分分别为 94 和 95,表明患者群体具有相似的功能。PS 组的平均膝关节屈曲度(114°)明显高于 CR 组(102°)( = 0.009)。在 CR 组中,运动适应测试(脚趾向上/脚趾向下)的表现更好( = 0.034)。在稳定性极限测试的轴向速度参数(线性目标定向)中,PS 组患者的表现更为成功( = 0.035)。
对于有较高跌倒风险的患者,建议使用 CR 植入物,因为它们具有更好的运动适应性,能够对快速表面变化做出快速反应。PS 组的线性目标定向更好,在计划适合相关活动的理想植入物时,应考虑到能够更快地朝预期方向移动。
