Abstract

PURPOSE

To compare the effects of upper extremity constraints on functional and dynamic postural control in children with hemiplegic cerebral palsy.

MATERIALS AND METHODS

Twelve children with hemiplegic cerebral palsy and 12 typically developing children ages 5 to 12 years (GMFCS I-II) were evaluated with the Timed Up and Go (TUG) and Biodex Balance System during three upper extremity (UE) conditions: 1) Free UEs (no constraints), 2) Simple UE constraint (unaffected/dominant UE constrained with a sling), and 3) Difficult UE constraint (Simple constraint plus the other UE holding cup of water).

RESULTS

The UE condition had significant effects on Overall Stability Index (OSI) (F = 24.899, < .001), Medial-Lateral Stability Index (MLSI) (F = 4.380, = .018), Anterior-Posterior Stability Index (F = 6.187, = .004), and TUG scores (F = 113.372, < .001). Group was significant for OSI (F = 7.906, = .010), MLSI (F = 13.113, = .002), and TUG (F = 36.282, < .001).

CONCLUSIONS

The upper extremity appears to have a role in maintaining functional balance and postural stability in children with hemiplegic cerebral palsy and should be considered during intervention programs.

目的

比较上肢约束对偏瘫脑瘫儿童功能性和动态姿势控制的影响。

材料与方法

共纳入 12 名偏瘫脑瘫儿童和 12 名年龄在 5 至 12 岁(GMFCS I-II)的正常发育儿童。使用计时起立行走测试(TUG)和 Biodex 平衡系统在三种上肢(UE)条件下对其进行评估:1)UE 自由(无约束),2)简单 UE 约束(非受累/优势 UE 用吊带约束),3)困难 UE 约束(简单约束+另一只 UE 持杯水)。

结果

UE 条件对总体稳定性指数(OSI)(F=24.899, <.001)、内外稳定性指数(MLSI)(F=4.380, =.018)、前后稳定性指数(F=6.187, =.004)和 TUG 评分(F=113.372, <.001)有显著影响。组对 OSI(F=7.906, =.010)、MLSI(F=13.113, =.002)和 TUG(F=36.282, <.001)也有显著影响。

结论

上肢似乎在偏瘫脑瘫儿童的功能性平衡和姿势稳定性维持中发挥作用,在干预计划中应予以考虑。
