Immunoglobulin G4-related disease (IgG4-RD) is a systemic fibroinflammatory disease of unknown etiology and pathogenesis, which can affect all organs. The most commonly involved organs are the pancreas, hepatobiliary system, salivary glands, orbits and lymph nodes. Rarely, the thyroid, pituitary, aorta, lung and kidney may also be involved; mesenteric involvement is rare. The association of IgG4-RD with some rheumatological diseases is observed; while there are data in the literature about rheumatoid arthritis and Sjögren syndrome, but association with ankylosing spondylitis is rare. Anti-tumor necrosis factor alpha (anti-TNFα) treatment has been tried in refractory IgG4-RD cases; but this case is of great interest in terms of progressive presentation under treatment with etanercept, an anti-TNFα agent, which is also used for the treatment. But, IgG4-related mesenteritis with ankylosing spondylitis receiving anti-TNFα therapy, which has also been tried in the treatment of IgG4 disease, has not been reported in the literature. 免疫球蛋白 G4 相关疾病（IgG4-RD）是一种病因和发病机制不明的系统性纤维炎症性疾病，可影响所有器官。最常受累的器官是胰腺、肝胆系统、唾液腺、眼眶和淋巴结。甲状腺、垂体、主动脉、肺和肾脏也很少受累；肠系膜受累罕见。观察到 IgG4-RD 与一些风湿病之间存在关联；虽然有文献中关于类风湿关节炎和干燥综合征的数据，但与强直性脊柱炎的关联很少见。在难治性 IgG4-RD 病例中尝试了抗肿瘤坏死因子α（抗-TNFα）治疗；但在接受抗-TNFα 药物依那西普（也用于治疗）治疗时表现出进行性表现的情况下，该病例非常有趣。但是，在 IgG4 疾病的治疗中也尝试了抗 TNFα 治疗，但文献中尚未报道 IgG4 相关性肠系膜炎伴强直性脊柱炎。关键词：免疫球蛋白 G4 相关疾病、肠系膜炎、强直性脊柱炎。
