Hair thread tourniquet syndrome has been well described in the literature, with one of the rare presentations including resultant tourniquet of the penis. We describe an unusual case in an adult resulting in erosion of the urethra as well as the reconstructive and social challenges faced in an attempt to preserve the glans.
