Abstract

PURPOSE

Grade-dependent decrease of lipid storage in clear cell renal cell carcinoma (ccRCC) leads to morphology changes in HE sections. This study investigated the role of cytoplasmic features in frozen sections of ccRCC on prognosis using the digital pathology approach.

METHODS

We established an automatic pipeline that performed tumor region selection, stain vector normalization, nuclei segmentation, and feature extraction based on the pathologic data from Shanghai General Hospital and The Cancer Genome Atlas database. Extracted features were subjected to survival analysis.

RESULTS

Kurtosis of the cytoplasm in the hematoxylin channel was correlated with progression-free survival (HR 0.10, 95% CI: 0.04-0.24, =6.52 10) and overall survival (HR 0.11, 95% CI: 0.05-0.31, =1.72 10) in ccRCC, which outperformed other texture features in this analysis. Multivariate Cox regression analysis revealed that low kurtosis of cytoplasm in the hematoxylin channel was an independent predictor for a shorter progression-free survival time (=0.044) and overall survival time ( = 0.01). Kaplan-Meier survival analysis of progression-free survival and overall survival also showed a significantly worse prognosis in patients with low kurtosis of the cytoplasm in the hematoxylin channel (both < 0.0001). Lower kurtosis of cytoplasm in the hematoxylin channel was associated with higher pathologic grade, less cholesterol ester, and more mitochondrial DNA content.

CONCLUSION

Kurtosis of the cytoplasm in the hematoxylin channel predicts survival in clear cell renal cell carcinoma.

目的

透明细胞肾细胞癌(ccRCC)中脂质储存的分级依赖性降低导致苏木精-伊红(HE)切片出现形态学变化。本研究采用数字病理学方法探讨ccRCC冰冻切片中细胞质特征对预后的作用。

方法

我们基于上海交通大学医学院附属瑞金医院和癌症基因组图谱(TCGA)数据库的病理数据,建立了一个自动流程,用于进行肿瘤区域选择、染色向量归一化、细胞核分割和特征提取。对提取的特征进行生存分析。

结果

苏木精通道中细胞质的峰度与ccRCC的无进展生存期(风险比[HR]0.10,95%置信区间[CI]:0.04 - 0.24,P = 6.52×10⁻⁶)和总生存期(HR 0.11,95% CI:0.05 - 0.31,P = 1.72×10⁻⁴)相关,在该分析中其表现优于其他纹理特征。多变量Cox回归分析显示,苏木精通道中细胞质的低峰度是无进展生存期缩短(P = 0.044)和总生存期缩短(P = 0.01)的独立预测因素。无进展生存期和总生存期的Kaplan - Meier生存分析也显示,苏木精通道中细胞质峰度低的患者预后明显更差(两者P均<0.0001)。苏木精通道中细胞质较低的峰度与较高的病理分级、较少的胆固醇酯和较多的线粒体DNA含量相关。

结论
