Abstract

Hemophagocytic syndrome (HPS) is a rare disease in clinical practice, and there are often cases of delayed diagnosis. At present, researchers have applied F-FDG PET/CT in the differential diagnosis of HPS, but no consensus has been formed. Therefore, this study aims to systematically evaluate the application value of F-FDG PET/CT in the diagnosis of HPS patients. PubMed, Embase, Cochrane Library, Chinese National Knowledge Infrastructure (CNKI), Wangfang database (Wangfang), and Chinese Biomedical Network (CBM) were searched to collect the relevant studies of F-FDG PET/CT in the diagnosis of HPS. Data from the articles were screened and extracted for meta-analysis using Stata16.0 software. A total of 10 retrospective studies, including 300 patients, were included in this meta-analysis. The meta-analysis results showed that the pooled sensitivity was 0.82 (95% CI: 0.67-0.95), specificity was 0.72 (95% CI: 0.51-0.86), positive likelihood ratio was 2.89 (95% CI: 1.46-5.75), positive likelihood ratio was 0.25 (95% CI: 0.12-0.54), diagnostic odds ratio was 2.89 (95% CI: 1.46-5.75), and AUC was 0.84 (95% CI: 0.81-0.87). The SUVmax in the liver, spleen, lymph nodes, and bone marrow of HPS patients was greater than 2.5, and the SUVmax in the spleen, lymph nodes, and bone marrow of malignant HPS patients was higher than that of benign HPS patients. The difference was statistically significant ( < 0.05). According to the existing literature evidence, F-FDG PET/CT is an effective method for diagnosing HPS.

噬血细胞综合征(HPS)是临床实践中一种罕见的疾病,常常存在诊断延迟的情况。目前,研究人员已经将 F-FDG PET/CT 应用于 HPS 的鉴别诊断,但尚未形成共识。因此,本研究旨在系统评价 F-FDG PET/CT 在 HPS 患者诊断中的应用价值。检索 PubMed、Embase、Cochrane 图书馆、中国知网(CNKI)、万方数据库(Wangfang)和中国生物医学文献数据库(CBM),收集 F-FDG PET/CT 诊断 HPS 的相关研究。采用 Stata16.0 软件对纳入文献的数据进行筛选和提取,并进行荟萃分析。共纳入 10 项回顾性研究,共计 300 例患者。荟萃分析结果显示,汇总敏感度为 0.82(95%CI:0.670.95),特异度为 0.72(95%CI:0.510.86),阳性似然比为 2.89(95%CI:1.465.75),阴性似然比为 0.25(95%CI:0.120.54),诊断比值比为 2.89(95%CI:1.465.75),曲线下面积为 0.84(95%CI:0.810.87)。HPS 患者肝、脾、淋巴结及骨髓的 SUVmax 值均大于 2.5,恶性 HPS 患者的脾、淋巴结及骨髓 SUVmax 值均高于良性 HPS 患者,差异有统计学意义( < 0.05)。根据现有文献证据,F-FDG PET/CT 是诊断 HPS 的有效方法。
