Abstract

OBJECTIVE

To determine relationship between the therapeutic effect of repetitive transcranial magnetic stimulation (rTMS) on cognitive impairment and brain derived neurotrophic factor (BDNF), glial cell derived neurotrophic factor (GDNF) levels in treatment resistant depression (TRD).

STUDY DESIGN

Descriptive study.

PLACE AND DURATION OF STUDY

Psychiatry Clinic of Konya Beyhekim Training and Research Hospital in Turkey, between June and November 2019.

METHODOLOGY

The study included 33 TRD patients and 33 healthy subjects. Patients received a total of 20 sessions of rTMS treatment. Serum BDNF and GDNF levels were measured before and after rTMS treatment. Additionally, the severity of depression as well as cognitive functions were assessed at the baseline and after the treatment.

RESULTS

The rTMS treatment significantly improved depressive and cognitive symptoms in patients with TRD. Although the level of serum BDNF and GDNF increased after rTMS treatment, it was associated with the improvement in symptoms, but not significantly.

CONCLUSIONS

rTMS treatment contributes to the antidepressant effect by normalising serum BDNF and GDNF levels in patients with TRD. Adding rTMS to antidepressants is, therefore, an appropriate treatment option for depressive patients with cognitive impairment. Key Words: rTMS, BDNF, GDNF, Cognitive function.

探讨重复经颅磁刺激(rTMS)治疗抵抗性抑郁症(TRD)患者认知障碍的疗效与脑源性神经营养因子(BDNF)、胶质细胞源性神经营养因子(GDNF)水平的关系。

研究设计

描述性研究。

地点和研究时间

土耳其科尼亚 Beyhekim 培训与研究医院精神病学诊所,2019 年 6 月至 11 月。

方法

该研究纳入了 33 例 TRD 患者和 33 名健康对照者。所有患者均接受了总共 20 次 rTMS 治疗。在 rTMS 治疗前后测量血清 BDNF 和 GDNF 水平。此外,在基线和治疗后评估抑郁严重程度和认知功能。

结果

rTMS 治疗可显著改善 TRD 患者的抑郁和认知症状。尽管 rTMS 治疗后血清 BDNF 和 GDNF 水平升高,但与症状改善相关,但无统计学意义。

结论

rTMS 治疗通过使 TRD 患者的血清 BDNF 和 GDNF 水平正常化,有助于发挥抗抑郁作用。因此,对于伴有认知障碍的抑郁患者,在抗抑郁药物治疗的基础上加用 rTMS 是一种合适的治疗选择。关键词:rTMS、BDNF、GDNF、认知功能。
