[The resilience of the Hospital of Mali to COVID-19 in a context of penuries].

Abstract

AIM

The objective of this research is to report the strategies of resilience mobilized by the Hospital of Mali to face Coronavirus disease (COVID-19).

METHOD

The data collected within the hospital covered the first months of the pandemic (April to July 2020). A total of 32 semi-structured interviews and 53 observation sessions were conducted. Data analyses were based on a conceptual framework and were conducted using a deductive approach.

RESULTS

The results show that, due to the multiple effects of the COVID-19 such as the aggravation of staff penuries, the high workloads, the need to create dedicated infrastructures, the drastic decrease in revenue due to the drop in hospital's attendance, the hospital and its staff implemented multiple strategies (e.g., reduction or postponement of some expenses, requisition of facilities, recruitment of contractual staff and redeployment of some healthcare workers). Those strategies generally allowed to maintain patients access to care, although there were many restrictions for non-COVID-19 patients. The hospital was able to build absorptive resilience.

CONCLUSION

This qualitative research provides a better understanding of hospitals' resilience processes to the COVID-19 pandemic in a hospital setting. Lessons learned from this study should help hospitals managers to design more appropriate and effective responses to future health crises.

目的

本研究的目的是报告马里医院为应对冠状病毒病(COVID-19)而调动的适应力策略。

方法

在医院内收集的数据涵盖了疫情的头几个月(2020年4月至7月)。总共进行了32次半结构化访谈和53次观察会议。数据分析基于一个概念框架,并采用演绎法进行。

结果

结果表明,由于COVID-19的多重影响,如工作人员短缺加剧、工作量大、需要创建专用基础设施、医院就诊人数下降导致收入大幅减少,医院及其工作人员实施了多种策略(如减少或推迟一些开支、征用设施、招聘合同制工作人员以及重新部署一些医护人员)。尽管对非COVID-19患者有许多限制,但这些策略总体上使患者能够继续获得医疗服务。医院能够建立吸收性适应力。

结论
