Abstract

OBJECTIVE

To study the expression and clinical importance of CD4T, CD8T cells, and CD4T/CD8T cell percentage in gastric cancer (GC) patients.

METHODS

The blood count of CD4T and CD8T lymphocytes was ascertained via flow cytometry before surgery in 93 GC patients undergoing gastrectomy. The CD4T, CD8T, and Foxp3T lymphocytes in cancerous and normal adjacent tissues and the presence of PD-L1 in cancerous tissues were detected via immunohistochemistry. The link between the permeation of CD4T, CD8T lymphocytes in venous blood, and cancer and normal adjacent tissues was analyzed.

RESULTS

Lauren histotype, TNM stage, lymphatic/nervous invasion, and NLR level were all considerably associated with peripheral CD4T and CD8T cell levels, whereas CD8T lymphocytes were also associated with vascular invasion ( < 0.05). The CD4T lymphocyte counts, CD4T, and CD8T cell percentage in GC tissues were found to have been decreased when compared to normal adjacent tissues, whereas the CD8T and Foxp3T lymphocyte count was higher in GC tissues ( < 0.05). According to a Spearman analysis, the CD4T and CD8T cell counts in tumor tissues were positively related to the Foxp3T lymphocyte count ( < 0.05). Greater peripheral CD4T lymphocyte counts and increased level of CD4T/CD8T percentage corresponded with greater CD4T cell levels and increased CD4T/CD8T quantity in normal adjacent tissues. Higher levels of peripheral CD8T cells corresponded with higher quantities of CD8T cells in cancer tissues. A reduced CD4T lymphocyte count, together with a reduced CD4T/CD8T percentage in venous blood, was consistent with a diminished CD4T cell count along with a reduced CD4T/CD8T lymphocyte ratio in cancer and normal adjacent tissues.

CONCLUSION

The peripheral quantity of CD4T and CD8T lymphocytes in GC patients can partly reflect the infiltrating state of these lymphocytes in cancer and normal adjacent tissues and can preliminarily predict immunotherapy response to a certain extent.

研究胃癌(GC)患者中CD4T、CD8T细胞及CD4T/CD8T细胞百分比的表达及临床意义。

方法

对93例行胃切除术的GC患者术前通过流式细胞术测定CD4T和CD8T淋巴细胞计数。通过免疫组织化学检测癌组织及癌旁正常组织中的CD4T、CD8T和Foxp3T淋巴细胞以及癌组织中PD-L1的表达情况。分析静脉血中CD4T、CD8T淋巴细胞浸润与癌组织及癌旁正常组织之间的联系。

结果

Lauren组织学类型、TNM分期、淋巴/神经侵犯及中性粒细胞与淋巴细胞比值(NLR)水平均与外周血CD4T和CD8T细胞水平显著相关,而CD8T淋巴细胞也与血管侵犯有关(P<0.05)。与癌旁正常组织相比,GC组织中CD4T淋巴细胞计数、CD4T及CD8T细胞百分比降低,而CD8T和Foxp3T淋巴细胞计数升高(P<0.05)。Spearman分析显示,肿瘤组织中CD4T和CD8T细胞计数与Foxp3T淋巴细胞计数呈正相关(P<0.05)。外周血CD4T淋巴细胞计数越高及CD4T/CD8T百分比升高,对应癌旁正常组织中CD4T细胞水平越高及CD4T/CD8T数量增加。外周血CD8T细胞水平越高,对应癌组织中CD8T细胞数量越多。静脉血中CD4T淋巴细胞计数降低及CD4T/CD8T百分比降低,与癌组织及癌旁正常组织中CD4T细胞计数减少及CD4T/CD8T淋巴细胞比值降低一致。

结论

GC患者外周血中CD4T和CD8T淋巴细胞数量可部分反映这些淋巴细胞在癌组织及癌旁正常组织中的浸润状态,并在一定程度上可初步预测免疫治疗反应。
