A 78-year-old Japanese woman without any history of asthma or smoking presented prolonged cough. Laboratory data showed elevated serum CEA levels and a chest CT revealed a mass with abnormal uptake in the left lower lobe. One month later, the mass spontaneously regressed, and CEA levels improved. However, the symptoms progressed during the observation period without treatment. Chest radiograph findings revealed collapse of the right middle lobe, and was isolated from the mucous plugs; the patient was diagnosed with allergic bronchopulmonary mycosis (ABPM). Herein, we report the first case of ABPM caused by with elevated CEA levels and mimicking lung cancer.
一名78岁的日本女性,无哮喘或吸烟史,出现长期咳嗽。实验室检查数据显示血清癌胚抗原(CEA)水平升高,胸部CT显示左肺下叶有一个伴有异常摄取的肿块。一个月后,肿块自发消退,CEA水平有所改善。然而,在观察期内未经治疗症状仍有进展。胸部X线检查结果显示右中叶肺不张,且与黏液栓分离;该患者被诊断为变应性支气管肺曲霉菌病(ABPM)。在此,我们报告首例由CEA水平升高并酷似肺癌的ABPM病例。
