Abstract

OBJECTIVE

To investigate the correlation between serum total serum bilirubin (TSB) levels and globus pallidus-related metabolic indexes of proton magnetic resonance spectroscopy (1H-MRS) in the newborn with neonatal jaundice.

METHODS

50 children with neonatal jaundice admitted to our hospital from January 2019 to January 2021 were recruited and assigned to a mild condition group (TSB < 221 mol/L,  = 16), a moderate condition group (221 mol/L ≤ TSB < 3 42 mol/L,  = 18), and a severe condition group (342 mol/L ≤ TSB < 428 mol/L,  = 16) based on peak TSB. The differences in globus pallidus-related metabolic indexes of 1H-MRS between the groups were compared and their correlation with TSB levels was analyzed.

RESULTS

The three groups had comparable N-acetylaspartic acid (NAA)/creatine (Cr), choline (Cho)/Cr, lactic acid (Lac)/Cr, and ml/Cr levels ( > 0.05), while there were statistical differences in glutamine (Glx)/Cr levels ( < 0.05). The severe condition group showed the highest levels of neuron-specific enolase (NSE), creatine kinase-MB (CK-MB), and troponin (cTnl), followed by the moderate group, and then the mild group ( < 0.05). The TSB level is positively correlated with the 1H-MRS metabolic index Glx/Cr.

CONCLUSIONS

The serum TSB level is correlated with the 1H-MRS metabolic index Glx/Cr in the newborn with neonatal jaundice, and the levels of TSB and Glx/Cr provide a reference for the diagnosis of bilirubin encephalopathy.

目的

探讨新生儿黄疸患儿血清总胆红素(TSB)水平与苍白球质子磁共振波谱(1H-MRS)相关代谢指标之间的相关性。

方法

选取2019年1月至2021年1月我院收治的50例新生儿黄疸患儿,根据TSB峰值将其分为轻症组(TSB<221μmol/L,n = 16)、中症组(221μmol/L≤TSB<342μmol/L,n = 18)和重症组(342μmol/L≤TSB<428μmol/L,n = 16)。比较三组间1H-MRS苍白球相关代谢指标的差异,并分析其与TSB水平的相关性。

结果

三组间N-乙酰天门冬氨酸(NAA)/肌酸(Cr)、胆碱(Cho)/Cr、乳酸(Lac)/Cr及肌醇(ml)/Cr水平比较,差异无统计学意义(P>0.05),而谷氨酰胺(Glx)/Cr水平比较,差异有统计学意义(P<0.05)。重症组神经元特异性烯醇化酶(NSE)、肌酸激酶同工酶MB(CK-MB)及肌钙蛋白(cTnl)水平最高,其次为中症组,轻症组最低(P<0.05)。TSB水平与1H-MRS代谢指标Glx/Cr呈正相关。

结论

新生儿黄疸患儿血清TSB水平与1H-MRS代谢指标Glx/Cr具有相关性,TSB及Glx/Cr水平可为胆红素脑病的诊断提供参考。
