Abstract

It was to investigate the influence of perioperative dexmedetomidine (DEX) anesthesia on the prognosis of elderly patients with gastrointestinal tumor (GIT) surgery. 90 patients who underwent laparoscopic radical gastrectomy for GIT were included. They were randomly divided into the experimental group (45 cases) with DEX+general anesthesia, and the control group (45 cases) with epidural anesthesia+general anesthesia. The indicators after surgery were compared between the two groups. The mean arterial pressure (MAP) was 74.8 ± 3.5 mmHg and the heart rate (HR) was 52.7 ± 8.2 beats/min in the experimental group, significantly lower than those of the control group ( < 0.05). The Visual Analog Scale (VAS) scores of both groups decreased greatly associated to those before surgery ( < 0.05). The levels of cortisol (COR) and immune adhesion inhibitor (FEIR) in the experimental group were significantly dissimilar from those in the control group ( < 0.05). The tumor necrosis factor-alpha (TNF-) was 96.4 ± 21.8 ng/L in the experimental group, observably lower than that in the control group ( < 0.05). The postoperative diamine oxidase (DAO) and D-lactate (D-lac) were 62.4 ± 9.3 mol/mL and 33.8 ± 7.2 ng/L, respectively, in the experimental group, much lower than those in the control group ( < 0.05). There were also significant differences in the initial recovery of bowel sounds, defecation, and total length of hospital stay (LOHS) between the groups ( < 0.05). DEX anesthesia had ideal sedative and analgesic effects, improving the prognosis of patients during surgery, and shortening the LOHS. Thus, it deserved a clinical application value.

本研究旨在探讨围手术期右美托咪定(DEX)麻醉对老年胃肠肿瘤(GIT)手术患者预后的影响。选择 90 例行腹腔镜 GIT 根治术的患者,随机分为 DEX+全身麻醉的实验组(45 例)和硬膜外麻醉+全身麻醉的对照组(45 例),比较两组术后指标。实验组平均动脉压(MAP)为 74.8±3.5mmHg,心率(HR)为 52.7±8.2 次/分,明显低于对照组(<0.05)。两组视觉模拟评分(VAS)均较术前明显降低(<0.05)。实验组皮质醇(COR)和免疫黏附抑制因子(FEIR)水平与对照组明显不同(<0.05)。实验组肿瘤坏死因子-α(TNF-)为 96.4±21.8ng/L,明显低于对照组(<0.05)。实验组术后二胺氧化酶(DAO)和 D-乳酸(D-lac)分别为 62.4±9.3mol/mL 和 33.8±7.2ng/L,明显低于对照组(<0.05)。两组肠鸣音恢复、排便、总住院时间(LOHS)差异均有统计学意义(<0.05)。DEX 麻醉具有理想的镇静、镇痛效果,改善手术期间患者的预后,缩短 LOHS,具有临床应用价值。
