Eight patients who failed to achieve continence after postanal repair for idiopathic incontinence were treated by anterior perineoplasty. Six gained satisfactory continence. Failure of the original postanal repair was probably because the anterior part of the pelvic floor had not been sufficiently supported as indicated by defecography pre- and postoperatively. Anterior perineoplasty corrected this defect.
