One of the most easily misdiagnosed injuries, which can result in severe consequences in young patients, is a fracture involving the epiphyseal growth plate. The Salter and Harris type V fracture is a relatively uncommon injury resulting from a severe crushing force applied through the epiphysis to an area of the growth plate. It is most likely to occur in the region of the knee or ankle. The following is a case report of a comminuted compression fracture of the epiphysis of the fifth metatarsal head, with complete transverse midshaft fracture of the proximal phalanx of the fifth digit.
