Abstract

OBJECTIVE

To investigate the application and effect of the nurse-led "outpatient-ward-home" management model in the care of patients with chronic kidney disease (CKD).

METHODS

120 patients with CKD admitted to our hospital between April 2020 and April 2021 were selected as trial subjects. All patients were divided into experimental and control groups according to the random number table method, with 60 cases in each group. The patients in the experimental group implemented the nurse-led "outpatient-ward-home" management model; the patients in the control group were given routine care and telephone follow-up. The self-rating anxiety scale (SAS), self-rating depression scale (SDS), Pittsburgh Sleep Quality Index (PSQI) score, self-management scores, nutritional status, renal function indicators, and chronic renal failure quality of life scale (QLICD-CRF2.0) were compared between the two groups before and 6 months after the intervention.

RESULTS

Before the intervention, there were no statistically significant differences between the control and experimental groups in SAS, SDS, PSQI scores, self-management scores, QLICD-CRF2.0 scores, body mass index (BMI), prealbumin (PAb), albumin (ALB), serum creatinine (Scr), blood urea nitrogen (BUN), and glomerular filtration rate (GFR) levels ( > 0.05). After 6 months of intervention, SAS, SDS, PSQI scores, Scr, BUN, and GFR levels were lower in the experimental group than in the control group; self-management scores, QLICD-CRF2.0 scores, BMI, PAb, and ALB levels were higher in the experimental group than in the control group ( < 0.05).

CONCLUSION

The nurse-led "outpatient-ward-family" intervention model can improve the negative emotions and sleep disorders of CKD patients, enhance patients' self-management ability, and to a certain extent, slow down the disease process and improve the quality of life.

目的

探讨护士主导的“门诊-病房-家庭”管理模式在慢性肾脏病(CKD)患者护理中的应用及效果。

方法

选取2020年4月至2021年4月我院收治的120例CKD患者作为试验对象。所有患者根据随机数字表法分为试验组和对照组,每组60例。试验组患者实施护士主导的“门诊-病房-家庭”管理模式;对照组患者给予常规护理及电话随访。比较两组患者干预前及干预6个月后的自评焦虑量表(SAS)、自评抑郁量表(SDS)、匹兹堡睡眠质量指数(PSQI)评分、自我管理评分、营养状况、肾功能指标以及慢性肾衰竭生活质量量表(QLICD-CRF2.0)。

结果

干预前,对照组和试验组在SAS、SDS、PSQI评分、自我管理评分、QLICD-CRF2.0评分、体重指数(BMI)、前白蛋白(PAb)、白蛋白(ALB)、血清肌酐(Scr)、血尿素氮(BUN)及肾小球滤过率(GFR)水平方面差异无统计学意义(P>0.05)。干预6个月后,试验组的SAS、SDS、PSQI评分、Scr、BUN及GFR水平低于对照组;试验组的自我管理评分、QLICD-CRF2.0评分、BMI、PAb及ALB水平高于对照组(P<0.05)。

结论

护士主导的“门诊-病房-家庭”干预模式可改善CKD患者的负面情绪及睡眠障碍,增强患者自我管理能力,在一定程度上延缓疾病进程,提高生活质量。
