Abstract

OBJECTIVE

To study the value of rheumatoid factor (RF) levels in the risk assessment of rheumatoid arthritis (RA) and combined hypertension and diabetes mellitus (DM) and construct RA risk prediction and medical image applications from rheumatoid factor levels.

METHODS

A total of 249 RA patients who were treated in the First People's Hospital of Yunnan Province, and another 149 non-RA people were selected as the controls. The clinical data and the detection results of serum circulating RF_IgA, RF_IgG, and RF_IgM were collected. The receiver operating curve (ROC) and logistic regression were used to analyze the value of RF levels in the risk assessment of RA and combined hypertension and DM.

RESULTS

After adjusting for age, BMI, smoking, drinking, hypertension, and diabetes, logistic regression analysis showed that RF_IgA positive, RF_IgG positive, and RF_IgM positive were all independent risk factors for RA ( < 0.05). The area under the curve (AUC) of circulating RF_IgA, RF_IgG, and RF_IgM levels in predicting RA was 0.79 (95% CI: 0.74-0.83, < 0.001), 0.73 (95% CI: 0.68-0.78, < 0.001), and 0.87 (95% CI: 0.84-0.91, < 0.001), respectively. The AUC for predicting RA was 0.88 (95% CI: 0.85-0.92, < 0.001) when combined detection of circulating RF_IgA, RF_IgG, and RF_IgM levels in peripheral blood. After adjusting for age and sex, logistic regression analysis showed that RF_IgA positive, RF_IgG positive, and RF_IgM positive were not independent risk factors for DM in RA patients ( > 0.05).

CONCLUSION

The levels of serum circulating RF_IgA, RF_IgG, and RF_IgM are valuable indicators for predicting the risk of RA, but not for the risk of RA complicated with hypertension and DM.

目的

研究类风湿因子(RF)水平在类风湿关节炎(RA)风险评估中的价值,构建基于 RF 水平的 RA 风险预测和医学影像应用。

方法

选取云南省第一人民医院收治的 249 例 RA 患者和 149 例非 RA 患者作为对照,收集临床资料及血清循环 RF_IgA、RF_IgG、RF_IgM 检测结果,采用受试者工作特征曲线(ROC)和 logistic 回归分析 RF 水平对 RA 及合并高血压和糖尿病的风险评估价值。

结果

校正年龄、BMI、吸烟、饮酒、高血压、糖尿病后,logistic 回归分析显示 RF_IgA 阳性、RF_IgG 阳性、RF_IgM 阳性均为 RA 的独立危险因素(<0.05)。循环 RF_IgA、RF_IgG、RF_IgM 水平预测 RA 的曲线下面积(AUC)分别为 0.79(95%CI:0.74-0.83,<0.001)、0.73(95%CI:0.68-0.78,<0.001)和 0.87(95%CI:0.84-0.91,<0.001),联合检测外周血循环 RF_IgA、RF_IgG、RF_IgM 水平预测 RA 的 AUC 为 0.88(95%CI:0.85-0.92,<0.001)。校正年龄、性别后,logistic 回归分析显示 RF_IgA 阳性、RF_IgG 阳性、RF_IgM 阳性不是 RA 患者 DM 的独立危险因素(>0.05)。

结论

血清循环 RF_IgA、RF_IgG、RF_IgM 水平是预测 RA 发病风险的有价值指标,但不能预测 RA 合并高血压和糖尿病的风险。
