A case report of a 75 year old man with abdominal aortic aneurysm and horseshoe kidney is presented. The diagnosis of this unusual association was made prior to surgery and the aneurysmectomy was carried out without great problems, by insertion of a dacron tube combined with reimplantation of an accessory right renal artery. There was an uneventful postoperative recovery and outcome. Previous publications are reviewed. The authors insist of the importance of CT-Scan, intravenous pyelogram and aortography to establish a complete preoperative diagnosis of this rare association with anomalous vasculature and pyeloureteral pattern. The prognosis of an aneurysm in association with a horseshoe kidney should not be worse than that of routine resection of an ordinary aneurysm.
本文报告一例75岁男性患者,患有腹主动脉瘤并马蹄肾。在手术前确诊了这种罕见的关联,通过植入涤纶血管移植片并重新植入右侧副肾动脉,顺利完成了动脉瘤切除术,未出现重大问题。术后恢复顺利,预后良好。回顾了以往的文献。作者强调CT扫描、静脉肾盂造影和主动脉造影对于全面术前诊断这种伴有异常血管和肾盂输尿管形态的罕见关联的重要性。与马蹄肾相关的动脉瘤的预后不应比普通动脉瘤的常规切除术更差。
