Clinical outcomes of a novel porcine small intestinal submucosa patch for full-thickness hand skin defects: a retrospective investigation.

Abstract

OBJECTIVE

To investigate the clinical outcomes of a novel soft tissue repair patch (porcine small intestinal submucosa patch, SIS patch) in the treatment of full-thickness hand skin defects.

METHODS

From January 2017 to July 2019, 80 patients with hand soft tissue defects, who met the inclusion criteria, were retrospectively reviewed and divided into two groups. After debridement, patients in group A were treated with the novel SIS patch to cover the wound, and patients in group B were treated with autologous skin graft. The dimensions of skin defect area and healing outcome were evaluated and recorded. Scar assessment was carried out using Scar Cosmesis Assessment and Rating Scale (SCAR scale) at the last follow-up postoperation, and the recovery of wound sensation was assessed at the same time using British Medical Research Council (BMRC) grading of sensorimotor recovery. All the data were collected and statistically analyzed.

RESULTS

A total of 80 patients were enrolled in the study with 40 patients in each group. Four patients in group A and 5 patients in group B were excluded due to wound infection and lost to follow-up. There were 36 patients in group A and 35 patients in group B finally got follow-up postoperation with mean interval of 12.75 ± 5.61 months in group A and 14.11 ± 5.42 months in group B. The dimensions of skin defect area in group A ranged from 7.5 to 87.5 cm (mean 25.97 ± 18.66 cm) and in group B ranged from 7.5 to 86.25 cm (mean 33.61 ± 19.27 cm) which have no significant difference (P > 0.05). SCAR scale results of group A and group B were 10.98 ± 0.33 and 9.49 ± 0.35, respectively, and the difference was statistically significant (P < 0.05). BMRC grading results showed 6 cases of S4, 11 cases of S3+, 5 cases of S3, 6 cases of S2, 6 cases of S1 and 2 cases of S0 in group A, and 8 cases of S4, 10 cases of S3+, 7 cases of S3, 4 cases of S2, 5 cases of S1, and 1 case of S0 in group B, which had no significant difference between them (P > 0.05).

CONCLUSIONS

The novel SIS patch is an applicable biological material in the treatment of hand skin defect, which could achieve a better cosmetic appearance of the newborn skin tissue.

探讨新型软组织修复贴(猪小肠黏膜下层贴片,SIS 贴片)治疗全层手部皮肤缺损的临床效果。

方法

回顾性分析 2017 年 1 月至 2019 年 7 月符合纳入标准的 80 例手部软组织缺损患者,分为两组。A 组患者清创后采用新型 SIS 贴片覆盖创面,B 组患者采用自体皮片移植。评估并记录两组患者皮肤缺损面积及愈合效果。末次随访时采用瘢痕美观评估量表(SCAR 量表)进行瘢痕评估,并采用英国医学研究理事会(BMRC)感觉运动恢复分级同时评估创面感觉恢复情况。收集并统计所有数据。

结果

共纳入 80 例患者,每组 40 例。A 组有 4 例患者和 B 组有 5 例患者因伤口感染和失访而被排除。A 组最终有 36 例患者和 B 组有 35 例患者获得术后随访,A 组的平均随访时间为 12.75±5.61 个月,B 组为 14.11±5.42 个月。A 组皮肤缺损面积为 7.587.5cm(平均 25.97±18.66cm),B 组为 7.586.25cm(平均 33.61±19.27cm),差异无统计学意义(P>0.05)。A 组和 B 组的 SCAR 量表评分为 10.98±0.33 和 9.49±0.35,差异有统计学意义(P<0.05)。BMRC 分级结果显示,A 组有 6 例 S4、11 例 S3+、5 例 S3、6 例 S2、6 例 S1 和 2 例 S0,B 组有 8 例 S4、10 例 S3+、7 例 S3、4 例 S2、5 例 S1 和 1 例 S0,两组间差异无统计学意义(P>0.05)。

结论

新型 SIS 贴片是治疗手部皮肤缺损的一种可行的生物材料,可使新生皮肤组织具有更好的外观。
