To characterize changes of corneal nerve morphology and tear indices in patients with neurotrophic keratitis (NK) treated with recombinant human nerve growth factor (rhNGF).
In a prospective observational study, six patients (nine eyes) were locally treated with rhNGF. Visual acuity, corneal fluorescein staining score, the heights of the tear river, lipid layer thickness (LLT), tear ferning (TF) test, conjunctival impression cytology (CIC) examination, the densities of cornea subbasal nerve fibers were determined before and after treatment.
Compared with baseline, there was a significant difference in corneal fluorescence staining scores (<0.01); all patient corneal epithelial defects recovered completely within 8wk, but there was no significant improvement in the height of the tear river (=0.202). LLT was significantly increased when compared with baseline (=0.042); however, the function of conjunctival goblet cells and mucin content did not significantly improve using the TF test and CIC examination (=0.557, =0.539). After 8wk of treatment, the average corneal subbasal nerve fiber density increased significantly (<0.01), as did the number of corneal nerve fiber branches (=0.001).
RhNGF can increase the density of corneal subbasal nerve fibers, promote the healing of persistent corneal epithelial defects and corneal ulcers in patients with NK, also improving tear function partially.
表征接受重组人神经生长因子(rhNGF)治疗的神经营养性角膜炎(NK)患者角膜神经形态和泪液指标的变化。
在一项前瞻性观察研究中,6例患者(9只眼)接受rhNGF局部治疗。在治疗前后测定视力、角膜荧光素染色评分、泪河高度、脂质层厚度(LLT)、泪液蕨样变(TF)试验、结膜印迹细胞学(CIC)检查、角膜基底膜下神经纤维密度。
与基线相比,角膜荧光染色评分有显著差异(<0.01);所有患者角膜上皮缺损在8周内完全愈合,但泪河高度无显著改善(=0.202)。与基线相比,LLT显著增加(=0.042);然而,使用TF试验和CIC检查,结膜杯状细胞功能和黏蛋白含量未显著改善(=0.557,=0.539)。治疗8周后,角膜基底膜下神经纤维平均密度显著增加(<0.01),角膜神经纤维分支数量也显著增加(=0.001)。
RhNGF可增加角膜基底膜下神经纤维密度,促进NK患者持续性角膜上皮缺损和角膜溃疡的愈合,也可部分改善泪液功能。
