Serum free light chain (FLC) measurements are increasingly prominent for patients with plasma cell disorders (PCDs) in screening, prognostic stratification, and monitoring therapy responses. We aimed to develop a sensitive, reliable, and accurate method for diagnosing PCDs that can notably decrease the time and cost of current methods. Here, we present a novel approach for FLC measurement using immunoenrichment on micro-affinity chromatography in combination with matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF) mass spectrometry (MS) detection. In this study, serum free kappa (κ) and free lambda (λ) light chain (LC) levels in the serum of 105 patients were compared between the nephelometric serum FLC quantification and MALDI-TOF MS detection. Cohen's kappa coefficient between the MALDI-TOF MS-based method and the FLC assay revealed an almost perfect agreement in the case of normal (negative) results (κ = 0.92; 95% confidence interval (CI): 0.837 to 0.968) and a good agreement in the case of increased (positive) results (κ = 0.76; 95% CI: 0.608 to 0.870). In Spearman's correlation analysis, the best correlation was found between serum free κ/λ ratios (r = 0.628, 0.496 to 0.732; <0.0001). Our method showed sensitivity (92.5%) and specificity (76.3%) for discrimination between the κ/λ FLC ratio compared to the serum FLC assay. The proposed method can significantly contribute to diagnosing and monitoring PCDs as it can significantly be time-saving, cost-effective in FLC measurement.
血清游离轻链(FLC)检测在浆细胞疾病(PCD)患者的筛查、预后分层及治疗反应监测中日益重要。我们旨在开发一种灵敏、可靠且准确的PCD诊断方法,以显著减少现有方法的时间和成本。在此,我们提出一种利用微亲和色谱免疫富集结合基质辅助激光解吸/电离飞行时间(MALDI-TOF)质谱(MS)检测的FLC测量新方法。在本研究中,对105例患者血清中的血清游离κ链和游离λ链(LC)水平,采用散射比浊法血清FLC定量检测与MALDI-TOF MS检测进行了比较。基于MALDI-TOF MS的方法与FLC检测之间的Cohen's kappa系数显示,正常(阴性)结果时一致性几乎完美(κ = 0.92;95%置信区间(CI):0.837至0.968),升高(阳性)结果时一致性良好(κ = 0.76;95% CI:0.608至0.870)。在Spearman相关性分析中,血清游离κ/λ比值之间的相关性最佳(r = 0.628,0.496至0.732;<0.0001)。与血清FLC检测相比,我们的方法在区分κ/λ FLC比值时显示出敏感性(92.5%)和特异性(76.3%)。所提出的方法在诊断和监测PCD方面可做出显著贡献,因为它在FLC测量中可显著节省时间和成本。
