Abstract

BACKGROUND

The molecular subtype of IDH mut combined with MGMT meth in gliomas suggests a good prognosis and potential benefit from TMZ chemotherapy. The aim of this study was to establish a radiomics model to predict this molecular subtype.

METHOD

The preoperative MR images and genetic data of 498 patients with gliomas were retrospectively collected from our institution and the TCGA/TCIA dataset. A total of 1702 radiomics features were extracted from the tumour region of interest (ROI) of CE-T1 and T2-FLAIR MR images. Least absolute shrinkage and selection operator (LASSO) and logistic regression were used for feature selection and model building. Receiver operating characteristic (ROC) curves and calibration curves were used to evaluate the predictive performance of the model.

RESULTS

Regarding clinical variables, age and tumour grade were significantly different between the two molecular subtypes in the training, test and independent validation cohorts ( < 0.05). The areas under the curve (AUCs) of the radiomics model based on 16 selected features in the SMOTE training cohort, un-SMOTE training cohort, test set and independent TCGA/TCIA validation cohort were 0.936, 0.932, 0.916 and 0.866, respectively, and the corresponding F1-scores were 0.860, 0.797, 0.880 and 0.802. The AUC of the independent validation cohort increased to 0.930 for the combined model when integrating the clinical risk factors and radiomics signature.

CONCLUSIONS

radiomics based on preoperative MRI can effectively predict the molecular subtype of IDH mut combined with MGMT meth.

背景

胶质瘤中异柠檬酸脱氢酶(IDH)突变联合O6-甲基鸟嘌呤-DNA甲基转移酶(MGMT)甲基化的分子亚型提示预后良好且可能从替莫唑胺(TMZ)化疗中获益。本研究旨在建立一种预测该分子亚型的放射组学模型。

方法

回顾性收集了本机构及癌症基因组图谱(TCGA)/癌症影像存档与信息系统(TCIA)数据集中498例胶质瘤患者的术前磁共振成像(MRI)图像和基因数据。从对比增强T1加权像(CE-T1)和液体衰减反转恢复序列(T2-FLAIR)MRI图像的肿瘤感兴趣区(ROI)提取了总共1702个放射组学特征。采用最小绝对收缩和选择算子(LASSO)及逻辑回归进行特征选择和模型构建。采用受试者操作特征(ROC)曲线和校准曲线评估模型的预测性能。

结果
