Sinus floor elevation and immediate dental implantation are commonly performed to treat dentition defects in elderly patients. Targeted cognitive behavioral interventions (CBI) during the perioperative period can reduce pain and anxiety as well as improve sleep quality. This can lead to improvements in patient cooperation during follow-up treatment and enhance the overall efficacy of the surgery.
The study aimed to investigate the impact of a cognitive behavioral intervention method on perioperative pain, anxiety, and sleep quality in elderly patients undergoing sinus floor elevation and immediate dental implantation.
Forty patients who required the treatment at the Stomatology Clinic in our hospital between December 2018 and December 2022 were enrolled in this randomized controlled trial. The patients were randomly divided into two groups: a control group (n= 20), which received conventional treatment and care during the perioperative period, and an intervention group (n= 20), which received comprehensive behavioral intervention in addition to the conventional treatment and care during the perioperative period. The perioperative anxiety, pain, and sleep quality of the patients in both groups were evaluated. Anxiety was assessed using the Self-Rating Anxiety Scale (SAS), sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI), and pain was measured using the visual analog scale (VAS).
No statistically significant differences in SAS and PSQI were observed between the two groups at the initial visit; the values were significantly higher than those measured postoperatively. The SAS scores and PSQI of patients on days 0 and 7 post-surgery in the intervention group were significantly lower than those in the control group (P< 0.05).
Perioperative cognitive behavioral intervention can effectively improve anxiety, postoperative pain and sleep quality in elderly patients who have undergone sinus floor elevation and immediate dental implantation, thereby reducing the incidence of complications.
鼻窦底提升术和即刻种植术常用于治疗老年患者的牙列缺损。围手术期实施有针对性的认知行为干预(CBI)可以减轻疼痛和焦虑,改善睡眠质量。这可以提高患者在随访治疗期间的合作程度,并增强手术的整体疗效。

本随机对照试验纳入了 2018 年 12 月至 2022 年 12 月在我院口腔科就诊的 40 例需要接受治疗的患者。患者被随机分为两组:对照组(n=20),在围手术期接受常规治疗和护理;干预组(n=20),在围手术期接受综合行为干预。评估两组患者的围手术期焦虑、疼痛和睡眠质量。焦虑采用自评焦虑量表(SAS)评估,睡眠质量采用匹兹堡睡眠质量指数(PSQI)评估,疼痛采用视觉模拟评分(VAS)评估。
两组患者在初诊时的 SAS 和 PSQI 评分无统计学差异,均显著高于术后评分;干预组患者术后 0 天和 7 天的 SAS 评分和 PSQI 显著低于对照组(P<0.05)。
围手术期认知行为干预可有效改善接受鼻窦底提升和即刻种植术的老年患者的焦虑、术后疼痛和睡眠质量,从而降低并发症的发生率。
