Peritoneal involvement is quite common in systemic lupus erythematosus (SLE). However, significant ascites as the predominant manisfestations of SLE is extremely rare, and very few cases have been reported. We describe a patient with SLE whose disease first manifested as longstanding, painless and intractable ascites. The ascites preceded the diagnosis of SLE by 3 years. The diagnosis of SLE should be considered in patients with the so called "idiopathic" ascites. Once diagnosed, the ascites seem to respond promptly to corticosteroids, however, prior to their administration other potential causes, especially infections and malignancy, should be ruled out.
腹膜受累在系统性红斑狼疮（SLE）中相当常见。然而，以大量腹水为SLE主要表现的情况极为罕见，仅有极少数病例被报道。我们描述了一名SLE患者，其疾病最初表现为长期存在的、无痛性且难治性的腹水。腹水出现3年后才诊断出SLE。对于所谓“特发性”腹水的患者应考虑SLE的诊断。一旦确诊，腹水似乎对皮质类固醇迅速产生反应，然而，在给予皮质类固醇之前，应排除其他潜在病因，尤其是感染和恶性肿瘤。
