Penetrating cardiac trauma from gunshots is usually fatal. We describe the case of a 62-year-old male presenting with ST-segment elevation myocardial infarction. A retained bullet embedded into the left ventricle was identified incidentally along with a ventricular septal defect from a gunshot wound decades prior. The ventricular septal defect and retained bullet were managed conservatively. ().
