BACKGROUND/AIM: Serum markers to determine the histological grade of hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) are still limited. This study aimed to investigate if serum extra spindle pole bodies-like 1 (ESPL1) protein could reflect the histological grade of HBV-related HCC.
A total of 154 patients with HBV-related HCC were enrolled in the experimental group and 41 non-HBV-related patients in the control. Enzyme-linked immunosorbent assay was used to detect serum ESPL1 levels. The differences in serological ESPL1, alpha-fetoprotein (AFP), and des-gamma-carboxy prothrombin (DCP) were compared between the two groups. HCC tumor diameter was measured, and pathological examination was performed to compare the relationship between ESPL1, AFP, and DCP and tumor size and histological grade.
Serum AFP and DCP levels showed no significant difference between experimental group and control group, and increased when the tumor diameter increased but were not related to HCC histological grade. Serological ESPL1 levels were higher in the experimental group than those in the control group, and positively correlated with the histological grade. In the experimental group, tumor size and histological grade were almost independent (Kappa=0.000); patients with medium size tumors had the highest serum ESPL1 levels and the highest proportion of poorly differentiated carcinomas, whereas 75.6% of patients with small size tumors had moderately differentiated carcinomas and only 20% well differentiated carcinomas.
Serum ESPL1 can reflect the malignant degree of HBV-related HCC and is helpful in identifying small size HCC tumors.
背景/目的:用于确定乙型肝炎病毒(HBV)相关肝细胞癌(HCC)组织学分级的血清标志物仍然有限。本研究旨在探讨血清纺锤体极体外体样蛋白 1(ESPL1)蛋白是否可以反映 HBV 相关 HCC 的组织学分级。
共纳入 154 例 HBV 相关 HCC 患者作为实验组,41 例非 HBV 相关患者作为对照组。采用酶联免疫吸附试验检测血清 ESPL1 水平。比较两组间血清 ESPL1、甲胎蛋白(AFP)和脱γ-羧基凝血酶原(DCP)的差异。测量 HCC 肿瘤直径,并进行病理检查,比较 ESPL1、AFP 和 DCP 与肿瘤大小和组织学分级的关系。
实验组和对照组血清 AFP 和 DCP 水平无显著差异,且随肿瘤直径增大而升高,但与 HCC 组织学分级无关。实验组血清 ESPL1 水平高于对照组,且与组织学分级呈正相关。实验组中,肿瘤大小和组织学分级几乎独立(Kappa=0.000);中肿瘤大小患者的血清 ESPL1 水平最高,且低分化癌的比例最高,而小肿瘤大小患者中 75.6%为中分化癌,仅有 20%为高分化癌。
血清 ESPL1 可以反映 HBV 相关 HCC 的恶性程度,有助于识别小 HCC 肿瘤。
