The paper deals with modern results on the pathogenesis of acute respiratory infections (ARI) caused by viruses. The course of the disease depends on exogenic (viral) factors and intrinsic individual conditions. The earliest steps of infection are only understood by immunologic, biochemical and virologic considerations until the first, only by electronmicroscopy detectable structural lesions of cells, the mucosa and the lung tissue can be seen and finally more or less severe damages of the respiratory tract with bronchitis and pneumonia occur. At first this is considered from a general point of view for all ARI with regard to local pathologic changes of bronchial tree and lung, to doubtlessly existing viremias and general toxic and circulatory damages of the whole organism. Thereafter some typical but different findings caused by the most important viruses are shortly described.
本文论述了由病毒引起的急性呼吸道感染(ARI)发病机制的现代研究成果。疾病的病程取决于外源性(病毒)因素和个体内在状况。在通过电子显微镜首次观察到细胞、黏膜和肺组织仅有的结构损伤,最终出现或多或少伴有支气管炎和肺炎的呼吸道严重损伤之前,感染的最初阶段只能通过免疫学、生物化学和病毒学方面的考量来理解。首先,从总体角度对所有ARI进行分析,涉及支气管树和肺部的局部病理变化、无疑存在的病毒血症以及整个机体的全身毒性和循环系统损伤。此后,简要描述了由最重要的病毒引起的一些典型但不同的发现。
