The Rhesus (Rh) blood group is a significant and complicated biological system in humans. Incompatible transfusion or pregnancy with Rh antigens can lead to the production of alloantibodies, among which the anti-E antibody is prevalent. The relationship between Anti-E antibody and HLA-II gene polymorphism in Chinese pregnant women is worth exploring. Our aim in this study was to verify the correlation between HLA-II gene polymorphisms and RhE alloimmunization in pregnant Chinese women through HLA-II typing and DR-RhE structural prediction. In total, 94 anti-E-negative pregnant women and 103 anti-E-positive pregnant women were enrolled from Southwest China Second Hospital, and HLA-II genotyping was performed using next-generation sequencing. NetMHCpan software was used to predict the binding of E -derived anchoring peptides to HLA-DRB1 molecules. AlphaFold was used to analyze the differences in antigen presentation based on the structure of major histocompatibility complex peptides. The HLA-DRB109:01-DQA103:02-DQB103:03 haplotype showed a significant positive association with anti-E. One E-derived anchoring peptide (219FWPSVNSPL227) was predicted to bind to the HLA-DRB109:01 molecule. The interaction between the 60Ser of DR9 and 226pro of RhE comprised one hydrogen bond. This study demonstrated that HLA-II haplotypes are associated with allo-anti-E antibodies in pregnant women from Sichuan Province, China. The HLA-DRB109:01-DQA103:02-DQB103:03 phenotype may enhance the formation of anti-E alloantibodies, and the HLA-DRB109:01 molecule may play a key role in alloimmunity.
恒河猴(Rh)血型系统是人类重要且复杂的生物系统。Rh抗原不相容的输血或妊娠可导致同种抗体的产生,其中抗-E抗体较为常见。中国孕妇中抗-E抗体与HLA-II基因多态性的关系值得探讨。本研究的目的是通过HLA-II分型和DR-RhE结构预测,验证中国孕妇HLA-II基因多态性与RhE同种免疫之间的相关性。总共从西南医科大学附属第二医院招募了94例抗-E阴性孕妇和103例抗-E阳性孕妇,并使用下一代测序技术进行HLA-II基因分型。使用NetMHCpan软件预测E衍生的锚定肽与HLA-DRB1分子的结合。使用AlphaFold基于主要组织相容性复合体肽的结构分析抗原呈递的差异。HLA-DRB109:01-DQA103:02-DQB103:03单倍型与抗-E呈显著正相关。预测一种E衍生的锚定肽(219FWPSVNSPL227)与HLA-DRB109:01分子结合。DR9的60Ser与RhE的226pro之间的相互作用包含一个氢键。本研究表明,HLA-II单倍型与中国四川省孕妇的同种抗-E抗体有关。HLA-DRB109:01-DQA103:02-DQB103:03表型可能会增强抗-E同种抗体的形成,并且HLA-DRB109:01分子可能在同种免疫中起关键作用。
