Abstract

BACKGROUND

Recipient area scalp necrosis is considered a potential complication of hair transplantation, but has rarely been reported. A small number of patients have developed scalp necrosis after hair transplantation with the widely used Follicular unit excision (FUE) technique. There are no guidelines to prevent and manage this complication. The aim of this study was to provide an insight into the pathogenesis, prevention, and management of scalp necrosis following hair transplantation.

METHODS

From 2012 to 2021, among more than 10 000 patients who underwent hair transplantation, only three developed scalp necrosis in our clinical experience, besides, one patient transferred to our hospital because of scalp necrosis after undergoing hair transplantation. According to the disease etiology and patients' symptom, a combination of wound management and antimicrobial therapy was employed. This study was approved by the institutional ethics committee of Nanfang Hospital.

RESULTS

Of the four patients, three received timely treatment and had a good prognosis. Necrosis became confined and healed within 2-3 weeks. Grafts in the lesion area partially survived. In case 4, due to improper treatment at the early stage, the lesion developed extensively and deeply, which not only delayed wound healing, but also resulted in complete loss of grafts.

CONCLUSION

Preoperative prophylaxis, timely diagnosis, and immediate treatment of scalp necrosis can prevent serious complications and reduce morbidity after hair transplantation.

背景

受区头皮坏死被认为是毛发移植的潜在并发症,但很少有报道。少数患者在广泛使用毛囊单位提取(FUE)技术进行毛发移植后出现头皮坏死。目前尚无预防和处理这种并发症的指南。本研究旨在探讨毛发移植后头皮坏死的发病机制、预防和处理。

方法

在 2012 年至 2021 年间,在我们的临床经验中,超过 10000 名接受毛发移植的患者中,只有 3 名患者发生头皮坏死,此外,还有 1 名患者因接受毛发移植后头皮坏死转至我院。根据病因和患者症状,采用伤口管理和抗菌治疗相结合的方法。本研究得到了南方医院机构伦理委员会的批准。

结果

这 4 名患者中,有 3 名得到了及时治疗,预后良好。坏死在 2-3 周内局限并愈合。病变区域的移植体部分存活。在病例 4 中,由于早期治疗不当,病变广泛且深入发展,不仅延迟了伤口愈合,而且导致移植体完全丧失。

结论

术前预防、及时诊断和立即治疗头皮坏死可以预防毛发移植后的严重并发症和降低发病率。
