Abstract

To evaluate the effect of the implementation of in 2015 on the smoking prevalence in people aged ≥15 years in Beijing during 2014-2021, and explore factors associated with tobacco use behavior in local population. s Using a pooled cross-sectional design, data from Beijing Adult Tobacco Survey in 2014, 2016, 2019 and 2021 (4 surveys) were combined into one dataset. The 4 surveys used same multistage cluster sampling procedure. After complex survey weighting, multiple logistic regression models were constructed to analyze factors influencing smoking status. A total of 8 484, 9 372, 8 534 and 10 551 respondents were included in the surveys in 2014, 2016, 2019 and 2021, respectively. The smoking prevalence rate was 23.4%, 22.3%, 20.3% and 19.9%, respectively, in Beijing residents aged ≥15 years, exhibiting a linear declining trend (=0.010). Factors associated with current smoking in men were age 25-44 years (=2.22, 95%: 1.68-2.95) and 45-64 years, (=2.64, 95%: 2.06-3.39), educational level of high school (=0.69, 95%: 0.49-0.95) and undergraduate and above (=0.46, 95%: 0.33-0.63), and awareness of smoking causing stroke (=0.71, 95%: 0.61-0.81), and awareness of smoking causing lung cancer (=0.53, 95%: 0.42-0.66), the differences were significant (all <0.05). After controlling interfering factors, the current smoking prevalence in men in 2019 (=0.73, 95%: 0.63-0.87, <0.001) and 2021 (=0.72, 95%: 0.61-0.88, <0.001) were significantly lower than that in 2014. Factors associated with current smoking in women were living alone (=1.80, 95%: 1.33-2.44), educational level of undergraduate and above (=0.43, 95%: 0.27-0.69), other occupations except doctor and teacher (=8.54, 95%: 2.80-26.02) or being retired/unemployed (=9.39, 95%: 3.19-27.65), and awareness of smoking causing cardiovascular events (=0.58, 95%: 0.39-0.84), and awareness of smoking causing lung cancer (=0.54, 95%: 0.35-0.83), the differences were significant (all <0.05). No significant change in smoking status in women was found in 4 surveys. The smoking prevalence rate in men in Beijing has declined since the implementation of 5 years, indicating the effectiveness of legislative approach in tobacco control. Socio-demographic factors and the awareness level of tobacco harm could influence smoking status. Future tobacco control programs should target the people with lower education level, young men, women living alone, and those with occupations other than teachers/doctors or the unemployed/retired and include more comprehensive health education.

为评估2015年实施的[具体措施]对2014 - 2021年北京市15岁及以上人群吸烟率的影响,并探讨当地人群烟草使用行为的相关因素。采用汇总横断面设计,将2014年、2016年、2019年和2021年北京成人烟草调查(4次调查)的数据合并为一个数据集。这4次调查采用相同的多阶段整群抽样程序。经过复杂的调查加权后,构建多重逻辑回归模型来分析影响吸烟状况的因素。2014年、2016年、2019年和2021年的调查分别纳入了8484、9372、8534和10551名受访者。北京市15岁及以上居民的吸烟率分别为23.4%、22.3%、20.3%和19.9%,呈线性下降趋势(P = 0.010)。男性当前吸烟的相关因素为年龄25 - 44岁(P = 2.22,95%置信区间:1.68 - 2.95)和45 - 64岁(P = 2.64,95%置信区间:2.06 - 3.39)、高中文化程度(P = 0.69,95%置信区间:0.49 - 0.95)以及本科及以上文化程度(P = 0.46,95%置信区间:0.33 - 0.63),还有知晓吸烟会导致中风(P = 0.71,95%置信区间:0.61 - 0.81)以及知晓吸烟会导致肺癌(P = 0.53,95%置信区间:0.42 - 0.66),差异均有统计学意义(均P < 0.05)。在控制干扰因素后,2019年(P = 0.73,95%置信区间:0.63 - 0.87,P < 0.001)和2021年(P = 0.72,95%置信区间:0.61 - 0.88,P < 0.001)男性当前吸烟率显著低于2014年。女性当前吸烟的相关因素为独居(P = 1.80,95%置信区间:1.33 - 2.44)、本科及以上文化程度(P = 0.43,95%置信区间:0.27 - 0.69)、除医生和教师以外的其他职业(P = 8.54,95%置信区间:2.80 - 26.02)或退休/失业(P = 9.39,95%置信区间:3.19 - 27.65),以及知晓吸烟会导致心血管事件(P = 0.58,95%置信区间:0.39 - 0.84)和知晓吸烟会导致肺癌(P = 0.54,95%置信区间:0.35 - 0.83),差异均有统计学意义(均P <0.05)。4次调查中女性吸烟状况无显著变化。自[具体措施]实施5年来,北京市男性吸烟率有所下降,表明立法控烟措施有效。社会人口学因素和烟草危害知晓水平会影响吸烟状况。未来的控烟项目应针对文化程度较低者、年轻男性、独居女性以及教师/医生以外职业者或失业/退休者,并开展更全面的健康教育。
